Objective To compare the surgical efficacy of the modified Borsellino procedure and modi-fied Devine procedure for correction of concealed penis in children.Methods The clinical data of 96 children with concealed penis admitted from January 2018 to February 2024 in the Affiliated Hospital of Yangzhou Univer-sity were retrospectively analyzed.Among them,52 cases as the observation group,aged(7.83±3.13)years,with BMI of(19.11±3.34)kg/m2 and penis length of(14.55±1.86)mm preoperatively,admitted from January 2021 to February 2024 were conducted the modified Borsellino procedure with the assistance of a V-shaped incision for detachment and fixation.A total of 44 children as the control group,aged(8.01±3.32)years,with BMI of(18.92±3.53)kg/m2 and penis length of(14.72±2.88)mm preoperatively,admitted from January 2018 to December 2020 were conducted the modified Devine procedure involving resec-tion of the narrowed frenulum,detachment of the penile sheath,and two-point fixation at the penile root.The perioperative indexes of operation time,intraoperative blood loss volume,penile length,penile elongation length and postoperative complications were compared between the two groups.Results Compared to preoper-atively,the penis length was significantly improved in both groups postoperatively(P<0.001).Compared to the control group,both the lengths of penis and penile elongation were greater[(46.43±3.80)mm vs.(40.11±4.60)mm,(31.73±3.02)mm vs.(26.19±4.32)mm],but the operation time was longer[(85.76±26.81)min vs.(56.63±12.52)min]in the observation group,and the differences were statistically significant(P<0.05).There was no injury of dorsal penile vessels and nerves,and urethral injury of ventral penis in both groups.All children were followed up for 3 to 6 months.Postoperative complication was found in 5 cases of the observation group,including 3(5.77%)of incision infection,1(1.92%)of foreskin edema,and 1(1.92%)of subcu-taneous hematoma,without penile retraction,and was in 12 cases of the control group,including 5(11.36%)of foreskin edema,4(9.09%)of penile retraction,2(4.54%)of incision infection,and 1(2.27%)of subcutaneous hematoma.Both incidences of foreskin edema and penile retraction were higher in the control group than those in the observation group,and the difference was statistically significant(P<0.05).Conclusions Both the modified Borsellino operation and the modified Devine operation are safe and effective for the correction of concealed penis in children.The modified Devine operation is relatively simple to conduct,while the modified Borsellino operation can achieve a better penile elongation effect and a lower penile retraction incidence.The appropriate surgical procedure can be selected according to the individual situation.
