Comparison of the efficacy of autogenous iliac bone and allograft bone implantation in the treatment of Schatzker type Ⅱ tibial plateau fracture
Objective To compare the clinic efficacy of autogenous iliac bone and allograft bone grafting in the treatment of Schatzker type Ⅱ tibial plateau fracture.Methods A total of 74 patients with Schatzker typeⅡ tibial plateau fracture were divided into autogenous iliac bone group and allograft bone group according to different surgical bone grafting methods,with 37 cases in each group.Internal fixation of the proximal lateral tibial locking plate was used in both groups,and autologous iliac bone was implanted at the collapsed tibial plateau in the autologous iliac bone group,and allograft bone was implanted at the collapsed tibial plateau in the allograft bone group.Both groups were compared for surgical related indicators,the score of Hospital for Special Surgery(HSS)and knee flexion range of motion at 3 and 6 months after surgery,and the incidence of complications.Results The intraoperative blood loss of(62.70±3.34)ml in the autogenous iliac bone group was higher than that of(44.50±3.03)ml in the allograft bone group;the operative time of(86.50±5.87)min and the fracture healing time of(18.40±3.20)weeks in the autogenous iliac bone group were longer than those of(77.30±2.98)min and(14.80±2.29)weeks in allograft bone group;the differences were statistically significant(P<0.05).At 3 and 6 months after surgery,there was no statistically significant difference in HSS scores between the two groups(P>0.05).At 3 and 6 months after surgery,there was no statistically significant difference in knee flexion range of motion between the two groups(P>0.05).The incidence of complications in the allograft bone group and autogenous iliac bone group were 5.4%(2/37)and 8.1%(3/37),and the difference between the two groups was not statistically significant(P>0.05).Conclusion Allograft bone has advantages over autogenous iliac bone in terms of fracture healing time,operative time,and blood loss,but it has similar effects on postoperative knee joint function and knee flexion range of motion,indicating that both allograft bone and autogenous iliac bone have advantages in the treatment of schatkzer type Ⅱ tibial plateau fracture,and surgery can be performed according to the specific conditions of patients.
