Analysis of the clinical effect of addition and subtraction of Shengyang Yiwei Decoction on nutritional diarrhea in patients with severe neurological diseases
Objective To analyze the clinical effect of addition and subtraction of Shengyang Yiwei Decoction on nutritional diarrhea in patients with severe neurological diseases.Methods 80 patients with nutritional diarrhea due to severe neurological diseases were included in an observation group and a control group under the random number table,with 40 cases in each group.The control group was treated with probiotics,and the observation group was treated with addition and subtraction of Shengyang Yiwei Decoction.The diarrhea symptoms,severity,nutritional status and time to reach the nutritional target of the two groups were compared on the 3rd,7th and 14th day of treatment.Results On the 7th and 14th day of treatment,the number of stool and total amount of stool in the observation group were(2.34±0.51)times/d,(206.34±22.16)g and(2.15±0.46)times/d,(175.35±23.16)g,which were less than those of(3.95±0.46)times/d,(287.55±29.57)g and(3.65±0.57)times/d,(235.67±30.59)g in the control group;the perineal assessment tool(PAT)scores were(5.10±0.18)and(4.82±1.06)points in the observation group,which were lower than those of(6.32±0.22)and(6.02±1.16)points in the control group;the differences were statistically significant(P<0.05).On the 7th and 14th day of treatment,the scores of the Glasgow coma scale(GCS)in the observation group were(11.20±1.33)and(13.26±1.57)points,which were higher than those of(9.77±1.02)and(11.59±1.62)points in the control group;the acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score were(13.12±1.20)and(9.88±1.62)points in the observation group,which were lower than those of(15.12±1.26)and(13.64±1.57)points in the control group;the differences were statistically significant(P<0.05).The time to reach the nutritional target of(4.15±0.62)d in the observation group was shorter than that of(6.77±0.72)d in the control group,and the difference was statistically significant(P<0.05).On the 3rd day of treatment,there was no statistically significant difference in hemoglobin(Hb),albumin(ALB),total cholesterol(TC)and lymphocyte count between the two groups(P>0.05).On the 7th and 14th day of treatment,Hb,ALB and lymphocyte counts in the observation group were(121.54±7.22)g/L,(35.46±3.26)g/L,(1.58±0.21)×109/L and(132.56±8.16)g/L,(37.25±3.61)g/L,(1.63±0.23)×109,which were higher than those of(110.62±6.42)g/L,(32.05±3.17)g/L,(1.46±0.25)×109/L and(111.87±9.23)g/L,(32.34±3.26)g/L,(1.48±0.31)×109/L in the control group;TC were(2.28±0.40)and(2.01±0.54)mmol/L,which were lower than those of(2.55±0.39)and(2.25±0.51)mmol/L in the control group;the differences were statistically significant(P<0.05).Conclusion Addition and subtraction of Shengyang Yiwei Decoction has a definite therapeutic effect on patients with nutritional diarrhea due to severe neurological diseases and can be popularized.
