Study on clinical value of human neutrophil lipocalin and D-dimer in the early diagnosis of severe bacterial sepsis in children
Objective To explore the application value of human neutrophil lipocalin(HNL)and D-dimer(D-D)in the early diagnosis of severe bacterial sepsis in children.Methods 30 children with severe bacterial sepsis were selected as the study group,and 30 children with common bacterial sepsis during the same period were selected as control group A,and 30 children with common bacterial infection during the same period were selected as control group B.All three groups of children were tested for HNL,D-D,C-reactive protein(CRP),procalcitonin(PCT),white blood cell count(WBC),aspartate aminotransferase(AST),alanine aminotransferase(ALT)],blood urea nitrogen(BUN),and serum creatinine(Cr)levels at admission.The levels of HNL,D-D,CRP,PCT,WBC,AST,ALT,BUN and Cr in the three groups were compared.Results The levels of HNL,D-D,CRP,PCT,WBC of children in study group were(625.17±69.14)ng/ml,(7549.51±1532.75)ng/ml,(95.62±6.56)mg/L,(21.37±4.24)ng/ml,(14.83±1.79)×109/L,those of children in control group A were(371.42±42.37)ng/ml,(1032.28±201.63)ng/ml,(21.41±3.43)mg/L,(10.41±2.26)ng/ml,(15.78±2.03)×109/L,those of children in control B were(191.58±17.89)ng/ml,(382.16±56.58)ng/ml,(21.79±2.48)mg/L,(0.82±0.11)ng/ml,(15.37±1.44)×109/L.There was no significant difference in WBC level among the three groups(P>0.05).There was no significant difference in CRP level between control group A and control group B(P>0.05).The levels of HNL,D-D,CRP,PCT in the study group were higher than those in control group A and control group B,and the levels of HNL,D-D and PCT in control group A were higher than those in control group B.The difference was statistically significant(P<0.05).The levels of AST,ALT,BUN,Cr in the study group were(93.82±8.56)U/L,(193.67±11.50)U/L,(15.75±4.06)mmol/L,(179.26±16.35)μmol/L,those in control group A were(42.58±6.91)U/L,(34.78±4.13)U/L,(4.48±1.47)mmol/L,(77.40±11.76)μmol/L,and those in control group B were(29.77±3.72)U/L,(32.39±4.89)U/L,(3.71±1.28)mmol/L,(75.57±6.24)μmol/L.The levels of AST,ALT,BUN and Cr in study group were higher than those in control group A and control group B,and the AST level in control group A was higher than that in control group B.The difference was statistically significant(P<0.05).There were no significant differences in ALT,BUN and Cr levels between control group A and control group B(P>0.05).Conclusion HNL and D-D are helpful in the early differential diagnosis of severe bacterial sepsis,common bacterial sepsis,and common bacterial infection in children.
