Effect of low-dose ropivacaine combined with oxycodone in anesthesia of lower extremity fracture surgery in elderly patients
Objective To discuss the effect of low-dose ropivacaine combined with oxycodone in anesthesia of lower extremity fracture surgery in elderly patients.Methods The clinical data of 80 elderly patients undergoing surgical treatment for lower extremity fracture were retrospectively analyzed and divided into a control group and an observation group according to different anesthesia methods,with 40 cases in each group.The control group was anesthetised with conventional dose of ropivacaine,and the observation group was anesthetised with low dose of ropivacaine combined with oxycodone.The changes of hemodynamic indexes(systolic blood pressure,diastolic blood pressure,heart rate),anesthesia quality,postoperative pain,and occurrence of adverse reactions were compared between the two groups.Results At 5 and 30 min after anesthesia,the systolic blood pressure,diastolic blood pressure and heart rate decreased in both groups,but the systolic blood pressure,diastolic blood pressure and heart rate in the observation group were higher than those in the control group,and the change range was smaller than that in the control group(P<0.05).The excellent and good rate of anesthesia was 95.00%(38/40)in the observation group and 97.50%(39/40)in the control group,with no significant difference(P>0.05).The numerical rating scale(NRS)scores of the observation group were(2.82±0.52)and(3.37±0.44)points at 2 and 6 h after surgery,which were lower than(3.58±0.54)and(4.10±0.51)points of the control group(P<0.05).There was no significant difference in NRS score at 12 h after surgery between the two groups(P>0.05).The incidence of adverse reactions in the observation group was 10.00%(4/40),which was lower than 25.00%(10/40)in the control group,but there was no significant difference between the two group(P>0.05).Conclusion The application of low-dose ropivacaine combined with oxycodone in lower extremity fracture surgery in the elderly has achieved good anesthetic effect,and can maintain the stability of hemodynamics,improve the postoperative analgesia effect,and has high safety.
