Clinical observation of the combination of glucocorticoids and surgery in the treatment of granulomatous lobular mastitis
Objective To observe the clinical efficacy of glucocorticoids combined with surgery in the treatment of granulomatous lobular mastitis.Methods 94 patients with granulomatous lobular mastitis were divided into a control group and a study group according to random numerical table,with 47 cases in each group.The control group received surgical treatment,while the study group received glucocorticoid combined with surgery for treatment.Comparison was made on treatment effects,serum inflammatory factors[serum C-reactive protein(CRP),interleukin-6(IL-6),interleukin-2(IL-2)]levels before and after treatment,surgical conditions,and patients'satisfaction with postoperative breast appearance,recurrence rate between the two groups.Results The total effective rate of the study group was 97.87%,which was higher than the control group's 80.85%(χ2=7.162,P=0.007<0.05).After treatment,the study group had IL-2 of(8.21±1.38)ng/ml,CRP of(8.03±2.13)mg/L,and IL-6 of(102.31±10.92)ng/ml,which were lower than(12.99±2.04)ng/ml,(12.48±3.44)mg/L,and(148.92±18.34)ng/ml in the control group(t=13.305,7.540,14.970;P<0.05).The study group had surgical time of(50.49±15.82)min,intraoperative bleeding volume of(21.30±10.39)ml,incision healing time of(4.01±1.03)d,and hospitalization time of(5.47±1.38)d,which were less than(58.39±15.36)min,(28.93±14.39)ml,(5.72±1.24)d,and(8.89±1.47)d in the control group(t=2.456,2.947,7.272,11.629;P<0.05).The satisfaction rate with postoperative breast appearance in the study group was 95.74%,which was higher than 80.85%in the control group(χ2=5.045,P<0.05).After half a year of follow-up,1 case relapsed in the study group,and the recurrence rate was 2.13%,while 8 cases relapsed in the control group,and the recurrence rate was 17.02%.The recurrence rate of the study group was lower than that of the control group(P<0.05).Conclusion The combination of glucocorticoids and surgery has a significant effect on granulomatous lobular mastitis,which can effectively reduce intraoperative bleeding volume,improving postoperative aesthetics,and have a low recurrence rate.
