Analysis of clinical and pathological features of gastrointestinal stromal tumors
Objective To investigate the clinical and pathological features of gastrointestinal stromal tumor(GIST).Methods 75 patients with gastrointestinal stromal tumor were selected as the study subjects,and all the data were recorded by retrospective analysis.The clinical features,pathological data,immunohistochemical results and CT characteristics of gastrointestinal stromal tumors were analyzed.Results Among the 75 patients with gastrointestinal stromal tumor,68.00%(51/75)aged>50 years;42.67%(32/75)had abdominal pain and tenderness or swelling discomfort;34.67%(26/75)had symptoms such as black stools and gastrointestinal bleeding;and 22.67%(17/75)had an abdominal mass.The patients with abdominal tenderness accounted for 33.33%(25/75);abdominal mass was detected in 36.00%(27/75)of patients;mass was detected by intestinal examination in 10.67%(8/75)of patients;patients with no obvious clinical symptoms or positive signs accounted for 20.00%(15/75).After laboratory tests,it was confirmed that 20.00%(15/75)of the patients had decreased albumin or total protein,38.67%(29/75)of the patients had formation of occult blood in the stool routine,and 41.33%(31/75)of the patients had decreased hemoglobin.Among 75 patients with gastrointestinal stromal tumors,10.67%(8/75)were pathologically graded as very low risk,22.67%(17/75)as low risk,24.00%(18/75)as intermediate risk,and 42.67%(32/75)as high risk.The cell morphology and histological features of gastrointestinal stromal tumor were characterized as spindle-shaped or epithelioid.In 75 patients with gastrointestinal stromal tumors,CD117 was positively expressed in up to 94.67%of patients,and CD34 was positively expressed in up to 72.00%of patients and some patients had positive expression of neuron-specific enolase(NSE),smooth muscle actin(SMA),S-100,and PGP915.Among the 75 patients with gastrointestinal stromal tumors,59 patients had CT findings of distended growth,presenting as ellipsoidal or spherical masses,and 28 patients showed lobulated CT morphology.Among the 34 patients,29 cases had relatively uniform mass density on plain CT scan,and the imaging of the lesion in 51 patients showed lesions such as hemorrhage,cystic changes and necrosis of different degrees,28 patients showed clear boundaries of the mass lesion,and 32 patients showed blurred boundaries of the mass and adherence to the surrounding related organs.No lesion calcification was found in 75 patients with gastrointestinal stromal tumor.The results of enhanced CT scan showed that there were 50 cases of uneven enhancement,17 cases of moderate enhancement,and 8 cases of mild enhancement.Conclusion Observing and analyzing the clinical characteristics,CT examination,and pathological features of gastrointestinal stromal tumors can provide a basis for the clinical diagnosis and treatment of gastrointestinal stromal tumors,which has important clinical research significance and value,and is worthy of clinical application.
