Application effect of ultrasound-guided transversus abdominis plane block in laparoscopic total extraperitoneal hernia repair
Objective To investigate the effect of ultrasound-guided transversus abdominis plane(TAP)block in laparoscopic totally extraperitoneal prosthetic(TEP).Methods A total of 100 patients with TEP were divided into a control group and an observation group according to the random number table method,each with 50 cases.The control group received general anesthesia,and the observation group received ultrasound-guided TAP block.Both groups were compared in terms of pain at different postoperative time points in the active and resting state;intraoperative opioid use,postoperative analgesic use;occurrence of nausea and vomiting and length of hospital stay.Results The visual analogue scale(VAS)scores in the active and resting state of the two groups decreased gradually at different postoperative time points;VAS scores in the observation group were(2.50±0.55),(1.69±0.52)and(1.22±0.33)points at 24,48 and 72 h postoperatively,and VAS scores in the resting state were(2.86±0.56),(2.30±0.66)and(1.96±0.33)points,which were lower than(2.76±0.45),(1.90±0.44),(1.38±0.41)and(3.55±0.88),(3.00±0.47),(2.63±0.94)points in the control group(P<0.05).The utilization rate of intraoperative opioids and postoperative analgesics were 6.00%and 4.00%in the observation group,which were lower than 20.00%and 16.00%in the control group(P<0.05).The incidence of postoperative nausea and vomiting in the observation group was 16.00%(8/50),which was significantly lower than 34.00%(17/50)in the control group;the length of hospital stay was(3.16±0.99)d in the observation group,which was shorter than(4.05±0.55)d in the control group(P<0.05).Conclusion Ultrasound-guided TAP can reduce the intraoperative and postoperative pain response of TEP,thereby reducing the use of opioids,reducing the risk of adverse reactions,shortening the length of hospital stay,and promoting the recovery of patients.
