观察地佐辛与瑞芬太尼复合丙泊酚用于无痛人工流产麻醉中的临床效果

目的 探析地佐辛与瑞芬太尼复合丙泊酚用于无痛人工流产麻醉中的临床效果。方法 选取 62 例接受无痛人工流产的患者,以随机原则分为对照组及观察组,各 31 例。对照组选择瑞芬太尼复合丙泊酚麻醉,观察组选择地佐辛复合丙泊酚麻醉。比较两组术后指标[术后疼痛数字评分法(NRS)评分、恢复定向力时间、意识清醒时间],麻醉效果,血流动力学指标[心率(HR)、平均动脉压(MAP)、血氧饱和度(SpO2)],不良反应发生情况。结果 观察组术后NRS评分(2。01±0。53)分低于对照组的(3。11±0。64)分(P<0。05);观察组恢复定向力时间、意识清醒时间与对照组比较,差异无统计学意义(P>0。05)。观察组麻醉效果Ⅰ级 27 例(87。10%),Ⅱ级 3 例(9。68%),Ⅲ级 1 例(3。23%);对照组麻醉效果Ⅰ级 20 例(64。52%),Ⅱ级 6 例(19。35%),Ⅲ级 5 例(16。13%)。观察组麻醉效果优于对照组(Z=2。125,P=0。034<0。05)。术中,观察组HR、MAP、SpO2 分别为(77。21±0。51)次/min、(79。71±0。32)mm Hg(1 mm Hg= 0。133 kPa)、(98。71±0。32)%,高于对照组的(70。22±0。25)次/min、(77。29±0。61)mm Hg、(95。29±0。13)%(P<0。05)。观察组不良反应发生率 6。45%低于对照组的 25。81%(χ2=4。292,P=0。038<0。05)。结论 与瑞芬太尼复合丙泊酚相比,地佐辛复合丙泊酚用于无痛人工流产整体麻醉效果更佳,可控制不良反应,可选。
Observation on clinical effect of dezocine and remifentanil combined with propofol on anesthesia for painless induced abortion
Objective To explore the clinical effect of dezocine and remifentanil combined with propofol on anesthesia for painless induced abortion.Methods 62 patients who received painless induced abortion were randomly divided into a control group and an observation group,with 31 cases in each group.The control group was anesthetized with remifentanil and propofol,while the observation group was anesthetized with dezocine and propofol.Comparison was made on postoperative indicatros[numerical rating scale(NRS)score,recovery time of orientation,recovery time of consciousness],anesthetic effect,hemodynamic indicators[heart rate(HR),mean arterial pressure(MAP),blood oxygen saturation(SpO2)]and adverse reactions between the two groups.Results The NRS score of(2.01±0.53)points in the observation group was lower than(3.11±0.64)points in the control group(P<0.05).There was no statistically significant difference(P>0.05)in the recovery time of orientation and consciousness between the observation group and the control group(P>0.05).In the observation group,the anesthesia effect was grade Ⅰ in 27 cases(87.10%),grade Ⅱ in 3 cases(9.68%),and grade Ⅲ in 1 case(3.23%).In the control group,the anesthesia effect was grade Ⅰ in 20 cases(64.52%),grade Ⅱ in 6 cases(19.35%),and grade Ⅲ in 5 cases(16.13%).The anesthetic effect of the observation group was better than that of the control group(Z=2.125,P=0.034<0.05).During surgery,HR,MAP and SpO2 in the observation group were(77.21±0.51)beats/min,(79.71±0.32)mm Hg(1 mm Hg= 0.133 kPa)and(98.71±0.32)%,which were higher than(70.22±0.25)beats/min,(77.29±0.61)mm Hg and(95.29±0.13)%in the control group(P<0.05).The incidence of adverse reactions in the observation group was 6.45%,which was lower than 25.81%in the control group(χ2=4.292,P=0.038<0.05).Conclusion Compared with remifentanil combined with propofol,diazocin combined with propofol has better overall anesthesia effect for painless induced abortion,can control adverse reactions,and is optional.
