Clinical application of combined spinal and epidural anesthesia plus quadratus lumborum block in tension-free hernioplasty of inguinal hernia
Objective To study the clinical effect of combined spinal and epidural anesthesia plus quadratus lumborum block in tension-free hernioplasty of inguinal hernia.Methods A total of 70 patients with inguinal hernia who underwent tension-free hernioplasty were selected as the study subjects,and they were divided into traditional group and modern group,with 35 cases in each group.Patients in the traditional group received combined spinal and epidural anesthesia,and patients in the modern group received combined spinal and epidural anesthesia plus quadratus lumborum block.The surgery-related conditions,pain conditions,heart rate,mean arterial pressure,and occurrence of adverse events of the two groups were compared.Results The onset of anesthesia,operation time,postoperative ambulation time,and hospitalization time of the patients in the modern group were(4.03±1.57)min,(44.05±10.05)min,(3.81±1.17)h,and(3.35±1.33)d,which were significantly shorter than(6.14±2.70)min,(59.03±13.18)min,(5.77±1.44)h,and(4.72±1.71)d of the traditional group.The difference was statistically significant(P<0.05).At 30 min postoperatively,the visual analogue scale(VAS)score of patients in the modern group was(2.35±0.73)points,which was significantly lower than(3.06±0.67)points in the traditional group,and the difference was statistically significant(P<0.05).At 15 and 30 min after anesthesia,the heart rate and mean arterial pressure of patients in the modern group were significantly lower than those in the traditional group,and the difference was statistically significant(P<0.05).The incidence rate of postoperative adverse events in the modern group was 2.86%,and that in the traditional group was 20.00%;the incidence rate of postoperative adverse events in the modern group was significantly lower than that in the traditional group(P<0.05).Conclusion Compared with the traditional combined spinal and epidural anesthesia,the combined spinal and epidural anesthesia plus quadratus lumborum block has better analgesic effect,higher safety and better postoperative recovery effect,which is worthy of clinical promotion.
