Analysis of the impact of clinical pharmacist guidance on rational medication and compliance in elderly patients with chronic diseases
Objective To explore the impact of clinical pharmacist guidance on rational medication and compliance in elderly patients with chronic diseases.Methods 130 elderly patients with chronic diseases were randomly divided into a control group and an experimental groups by computer,with 65 cases in each group.The control group received routine management,and the experimental group received clinical pharmacist guidance based on the control group.The Self-efficacy for Appropriate Medication Use Scale(SEAMS),Adherence to Refills and Medications Scale(ARMS)scores,adverse reactions,quality of life score,and readmission status were compared between the two groups.Results At 3 months after discharge,patients in both groups had higher SEAMS score and lower ARMS score than than those at admission;the experimental group had higher SEAMS score of(30.19±2.41)points than(26.25±2.01)points in the control group,and lower ARMS score of(13.94±0.94)points than(16.41±1.02)points in the control group(P<0.05).The incidence of adverse reactions during medication in the experimental group was 6.15%,which was lower than 20.00%in the control group(P<0.05).At 3 months after discharge,the scores of psychological field,physiological field and social field of the two groups were higher than those at admission;the scores of psychological field,physiological field and social field of the experimental group were(18.92±1.54),(19.14±2.53)and(18.01±1.78)points,which were higher than(14.84±1.46),(16.70±1.56)and(15.41±1.44)points of the control group(P<0.05).The readmission rate within 3 months after discharge in the experimental group was 1.54%,which was lower than 10.77%in the control group(χ2=4.795,P=0.029<0.05).Conclusion Using clinical pharmacist guidance for elderly patients with chronic diseases can improve rational medication efficacy and compliance,and prevent adverse reactions;At the same time,it can also improve the quality of life of patients and reduce the risk of readmission,which is worth promoting and applying.
