目的 探究分析无创呼吸机治疗重症支气管哮喘患者的临床效果。方法 96 例重症支气管哮喘患者,根据随机数字表法分为对照组和研究组,每组 48 例。对照组采取常规治疗,研究组在对照组基础上应用无创呼吸机辅助治疗。比较两组患者治疗效果、治疗前后血气指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)]、肺功能指标[第 1 秒用力呼气容积(FEV1)、第 1 秒用力呼气容积与用力肺活量比值(FEV1/FVC)及呼气峰值流速(PEF)]、生活质量评分。结果 研究组患者治疗总有效率 97。92%高于对照组的 81。25%(P<0。05)。治疗后,研究组患者PaO2 水平(91。63±4。85)mm Hg(1 mm Hg=0。133 kPa)高于对照组的(86。57±4。52)mm Hg,PaCO2 水平(43。62±2。17)mm Hg低于对照组的(46。58±2。09)mm Hg(P<0。001)。治疗后,研究组患者FEV1、FEV1/FVC及PEF水平分别为(1。81±0。42)L、(75。48±5。61)%、(5。22±1。46)L/s,高于对照组的(1。25±0。31)L、(69。51±4。58)%、(4。15±1。37)L/s(P<0。001)。治疗后,研究组患者生理功能、心理能力、社交能力、精神/信仰、环境状态评分分别为(66。93±4。79)、(59。39±3。78)、(49。38±3。22)、(55。81±4。82)、(58。73±4。53)分,高于对照组的(63。26±5。17)、(55。34±5。72)、(46。38±6。21)、(51。49±5。72)、(56。59±5。28)分(P<0。05)。结论 无创呼吸机治疗重症支气管哮喘患者疗效显著,并能够改善患者肺功能及血气指标,提高患者生活质量。
Analysis of the clinical effect of non-invasive ventilator on patients with severe bronchial asthma
Objective To explore and analyze the clinical effect of non-invasive ventilator on patients with severe bronchial asthma.Methods 96 patients with severe bronchial asthma were collected as the research subjects.According to the random number table method,the included subjects were randomly divided into a control group and a study group,with 48 cases in each group.The control group received conventional treatment,and the study group received non-invasive ventilator assisted treatment on the basis of the control group.Patients in both groups were compared in terms of treatment efficiency,blood gas index[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)],lung function index[forced expiratory volume in one second(FEV1),ratio of FEV1 to forced vital capacity(FEV1/FVC)and peak expiratory flow rate(PEF)],and quality of life score.Results The total effective rate of the study group was 97.92%,which was higher than 81.25%of the control group(P<0.05).After treatment,the study group had higher PaO2 of(91.63±4.85)mm Hg(1 mm Hg= 0.133 kPa)than(86.57±4.52)mm Hg in the control group,and lower PaCO2 of(43.62±2.17)mm Hg than(46.58±2.09)mm Hg in the control group(P<0.001).After treatment,FEV1,FEV1/FVC and PEF in the study group were(1.81±0.42)L,(75.48±5.61)%and(5.22±1.46)L/s,which were higher than(1.25±0.31)L,(69.51±4.58)%and(4.15±1.37)L/s in the control group(P<0.001).After treatment,the scores of physiologic function,mental ability,social ability,spirit/belief and environmental status of patients in the study group were(66.93±4.79),(59.39±3.78),(49.38±3.22),(55.81±4.82)and(58.73±4.53)points,which were higher than(63.26±5.17),(55.34±5.72),(46.38±6.21),(51.49±5.72)and(56.59±5.28)points in the control group(P<0.05).Conclusion Non-invasive ventilator is effective in the treatment of severe bronchial asthma,and can improve the lung function and blood gas indexes of patients,so as to improve the quality of life of patients.
