目的 探讨支气管镜肺泡灌洗联合高频振荡排痰机在大叶性肺炎患儿中的临床治疗效果及应用价值。方法 60 例大叶性肺炎患儿为研究对象,采用简单化随机法将患儿分为对照组(30 例)和观察组(30 例)。对照组患儿实施常规对症治疗联合高频振荡排痰机,观察组在对照组基础上增加支气管镜肺泡灌洗进行治疗。比较两组患儿的临床疗效、肺功能指标(最大呼气中段平均流速、第 1 秒用力呼气容积和用力肺活量)、血清炎症指标[降钙素原(PCT)、C反应蛋白(CRP)]和不良事件发生率。结果 与对照组(80。0%)比较,观察组临床总有效率(96。7%)明显升高(P<0。05)。与治疗前比较,治疗后两组最大呼气中段平均流速、第 1 秒用力呼气容积和用力肺活量均有所升高(P<0。05);治疗后观察组患儿的最大呼气中段平均流速(1。92±0。38)L/s、第 1 秒用力呼气容积(1。85±0。33)L、用力肺活量(3。54±0。67)L高于对照组的(1。32±0。31)L/s、(1。27±0。29)L、(2。15±0。48)L(P<0。05)。与治疗前比较,治疗后两组血清CRP和PCT水平均有所降低(P<0。05);治疗后观察组患儿血清CRP(2。27±0。19)mg/L、PCT(0。08±0。02)ng/ml明显低于对照组的(6。68±0。85)mg/L、(0。16±0。05)ng/ml(P<0。05)。两组患儿不良事件发生率比较无统计学差异(P>0。05)。结论 支气管镜肺泡灌洗联合高频振荡排痰机在大叶性肺炎患儿中的临床疗效显著,能有效改善患儿的肺功能,降低患儿血清中炎症因子水平,且安全性良好,值得临床推广应用。
The application value of bronchoscopic alveolar lavage combined with high-frequency chest wall oscillation in the treatment of lobar pneumonia
Objective To explore the clinical effect and application value of bronchoscopic alveolar lavage combined with high-frequency chest wall oscillation devices in the treatment of children with lobar pneumonia.Methods 60 children with lobar pneumonia were divided into a control group(30 cases)and an observation group(30 cases)by simple randomization method.The control group received conventional symptomatic treatment combined with high-frequency chest wall oscillation devices,and the observation group received bronchoscopic alveolar lavage on the basis of the control group.Patients in both groups were compared in terms of clinical efficacy,pulmonary function indexes(maximum mid-expiratory flow,forced expiratory volume in one second and forced vital capacity),serum inflammatory markers[procalcitonin(PCT),C-reactive protein(CRP)]and incidence of adverse events.Results Compared with the control group(80.0%),the total clinical effective rate of the observation group(96.7%)was significantly increased(P<0.05).Compared with before treatment,the maximum mid-expiratory flow,forced expiratory volume in one second and forced vital capacity in both groups were increased after treatment(P<0.05).After treatment,the maximum mid-expiratory flow in the observation group was(1.92±0.38)L/s,the forced expiratory volume in one second was(1.85±0.33)L,and the forced vital capacity was(3.54±0.67)L,which were higher than(1.32±0.31)L/s,(1.27±0.29)L,and(2.15±0.48)L in the control group(P<0.05).Compared with before treatment,the serum CRP and PCT levels in both groups were decreased after treatment(P<0.05).After treatment,the observation group had serum CRP of(2.27±0.19)mg/L and PCT of(0.08±0.02)ng/ml,which were significantly lower than(6.68±0.85)mg/L and(0.16±0.05)ng/ml in the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion Bronchoscopic alveolar lavage combined with high-frequency oscillatory expectoration has significant clinical efficacy in children with lobar pneumonia,which can effectively improve lung function and reduce the level of inflammatory factors of children.It is safe and worthy of clinical promotion and application.
