摘要
目的 探讨自拟痤疮汤配合粉刺去除术治疗痤疮的临床疗效.方法 92 例痤疮患者,随机分为对照组和观察组,每组 46 例.对照组使用粉刺去除术治疗,观察组在对照组基础上使用自拟痤疮汤治疗.比较两组临床疗效、痤疮症状积分、各项评分[痤疮严重程度评分量表(GAGS)、痤疮瘢痕临床评分量表(ECCA)、痤疮伤残指数量表(ADI)、皮肤病生活质量指数(DLQI)]、各项炎性因子[血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)、γ-干扰素(IFN-γ)].结果 观察组治疗总有效率为 95.65%,明显高于对照组的 73.91%(P<0.05).观察组治疗后皮损数量、皮损颜色、皮损程度、皮损肿痛痤疮症状积分分别为(1.61±0.40)、(1.68±0.56)、(1.65±0.37)、(0.30±0.07)分,均低于对照组的(2.15±0.46)、(2.19±0.62)、(2.43±0.46)、(1.02±0.16)分(P<0.05).观察组治疗后GAGS评分(7.90±1.43)分、ECCA评分(44.13±5.37)分、ADI评分(5.28±1.26)分、DLQI评分(3.47±0.56)分均低于对照组的(9.81±1.69)、(51.95±6.10)、(7.63±1.85)、(4.65±0.75)分(P<0.05).观察组治疗后血清TNF-α(94.68±14.52)pg/ml、IL-1β(85.23±13.37)pg/ml、IL-8(35.70±4.62)pg/ml低于对照组的(105.83±15.26)、(92.86±14.08)、(42.98±4.52)pg/ml,IFN-γ(1005.64±77.18)pg/ml高于对照组的(968.25±83.43)pg/ml(P<0.05).结论 自拟痤疮汤配合粉刺去除术治疗痤疮的临床疗效显著,能有效减轻皮损,缓解症状,降低炎症反应,改善皮肤状态,提高生活质量.
Abstract
Objective To explore the clinical efficacy of self-made acne decoction combined with acne removal in the treatment of acne.Methods 92 patients with acne were randomly divided into a control group and an observation group,each with 46 cases.The control group was treated with acne removal,and on this basis,the observation group was treated with self-made acne decoction.Comparison was made on clinical efficacy,acne symptom score,various scores[global acne grading system(GAGS),echelle d'evaluation clinique des cicatrices d'acne(ECCA),acne disability index(ADI),dermatology quality of life index(DLQI)]and inflammatory factors[serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-8(IL-8),gamma interferon(IFN-γ)]between the two groups.Results The total effective rate of the observation group was 95.65%,which was significantly higher than 73.91%of the control group(P<0.05).After treatment,the scores of number,color,degree and swelling pain in the observation group were(1.61±0.40),(1.68±0.56),(1.65±0.37)and(0.30±0.07)points,which were lower than(2.15±0.46),(2.19±0.62),(2.43±0.46)and(1.02±0.16)points in the control group(P<0.05).After treatment,the observation group had GAGS score of(7.90±1.43)points,ECCA score of(44.13±5.37)points,ADI score of(5.28±1.26)points and DLQI score of(3.47±0.56)points,which were all lower than(9.81±1.69),(51.95±6.10),(7.63±1.85)and(4.65±0.75)points in the control group(P<0.05).After treatment,the observation group had serum TNF-α of(94.68±14.52)pg/ml,IL-1β of(85.23±13.37)pg/ml,and IL-8 of(35.70±4.62)pg/ml,which were lower than(105.83±15.26),(92.86±14.08),and(42.98±4.52)pg/ml in the control group;IFN-γ was(1005.64±77.18)pg/ml in the observation group,which was higher than(968.25±83.43)pg/ml in the control group(P<0.05).Conclusion Self-made acne decoction combined with acne removal is effective in treating acne,which can effectively reduce skin lesions,relieve symptoms,reduce inflammatory reaction,improve skin condition and improve quality of life of patients.
