Clinical effect of proximal femoral nail anti-rotation in the treatment of intertrochanteric fracture of the femur in advanced age
Objective To analyze the value of proximal femoral nail anti-rotation in the treatment of intertrochanteric fracture of the femur in advanced age.Methods 80 elderly patients with intertrochanteric fracture of the femur were divided into an observation group and a control group according to random number table method,with 40 cases in each group.The control group was treated with conventional intramedullary nail,while the observation group was treated with proximal femoral nail anti-rotation.Comparison was made on surgical indexes(incision length,operation time,intraoperative blood loss),fracture healing time,postoperative weight-bearing time,Harris score of hip function and score of Fugl-Meyer assessment(FMA)of the lower extremity score at different time points between the two groups.Results The observation group had shorter incision length of(4.24±0.36)cm than(13.51±0.11)cm in the control group;the operation time of the observation group was(108.56±12.15)min,the fracture healing time was(12.26±1.11)weeks and the weight-bearing time was(12.26±1.57)weeks,which were shorter than(134.16±11.51)min,(16.58±1.74)weeks and(16.62±1.52)weeks in the control group;the observation group had less intraoperative blood loss of(152.16±14.15)ml than(219.65±13.68)ml in the control group(P<0.05).The Harris score of hip function of the observation group were(66.52±2.14),(81.56±1.11)and(86.25±1.27)points at 3 months,6 months and 1 year after treatment,which were significantly higher than(61.28±3.26),(72.26±2.46)and(79.85±2.46)points of the control group(P<0.05).The scores of FMA of the lower extremity scores of the observation group were(18.62±2.13),(24.13±2.41)and(26.63±1.27)points at 3 months,6 months and 1 year after treatment,which were significantly higher(15.24±1.39),(20.16±2.11)and(22.13±2.17)points of the control group(P<0.05).Conclusion The treatment of intertrochanteric fracture of the femur with proximal femoral nail anti-rotation can significantly improve the therapeutic effect and shorten the postoperative rehabilitation time,which plays an important role in improving the function of hip joint and lower extremity,and is worthy of widespread clinical application.
