Analysis of the relationship between intraoperative blood transfusion and deep venous thrombosis in patients undergoing cardiac surgery
Objective To analyze the relationship between intraoperative blood transfusion and deep venous thrombosis in patients undergoing cardiac surgery.Methods 170 patients who underwent cardiac surgery were categorized into a control group(no occurrence,140 patients)and a study group(occurrence,30 patients)based on the occurrence of deep vein thrombosis after surgery.Both groups were compared in terms of general data,laboratory indexes[total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),uric acid(UA),glycosylated hemoglobin(HbA1c),prothrombin time(PT),activated partial thromboplastin time(APTT),D-dimer,and platelet counts,fibrinogen(FIB)].The factors causing postoperative deep vein thrombosis after cardiac surgery were analyzed,as well as the correlation between intraoperative blood transfusion and each factor.Results The duration of cardiac disease in the study group was longer than that in the control group;the percentages of hypertension,diabetes mellitus,smoking,alcohol consumption,intraoperative blood transfusion,operation time>120 min,and postoperative hospitalization time≥7 d in the study group were higher than those in the control group;the percentages of antithrombotic pumps and intraoperative antibiotic use in the study group were lower than those in the control group(P<0.05).The study group had TC of(3.90±0.61)mmol/L,TG of(2.20±0.44)mmol/L,UA of(398.52±66.33)μmol/L,FIB of(3.22±0.63)g/L,D-dimer of(3.52±0.19)mg/L,and platelet count of(254.11±38.66)×109/L,which were higher than(3.48±0.74)mmol/L,(1.89±0.39)mmol/L,(347.51±69.21)μmol/L,(3.07±0.21)g/L,(3.19±0.22)mg/L,and(223.33±34.19)×109/L in the control group;HDL-C of(0.79±0.29)mmol/L in the study group was lower than(1.15±0.44)mmol/L in the control group;the study group had PT of(10.33±2.21)s and APTT of(31.58±4.25)s,which were shorter than(14.22±2.33)and(36.90±4.66)s in the control group(P<0.05).Multivariate Logistic regression analysis showed that diabetes mellitus,hypertension,non-use of antithrombotic pump and intraoperative blood transfusion were the risk factors for deep vein thrombosis(P<0.05).Spearman correlation analysis showed that intraoperative blood transfusion was positively correlated with diabetes mellitus,hypertension,and negatively correlated with the use of antithrombotic pump(P<0.05).Conclusion Combined diabetes mellitus,hypertension,non-use of antithrombotic pumps,and intraoperative blood transfusion are risk factors for postoperative deep vein thrombosis after cardiac surgery.Intraoperative blood transfusion is positively correlated with diabetes mellitus,hypertension,and negatively correlated with the use of antithrombotic pump.Medical staff need to identify high-risk individuals for deep vein thrombosis as early as possible,provide targeted guidance to patients,reduce the risk of deep vein thrombosis,and promote their postoperative recovery.
