Clinical value of serum CRP,PCT and IL-6 in early diagnosis of hospital-acquired pneumonia
Objective To observe the value and efficacy of serum C-reactive protein(CRP),procalcitonin(PCT)and interleukin-6(IL-6)in early diagnosis of hospital-acquired pneumonia.Methods 40 patients with hospital-acquired pneumonia were selected as the observation group,and another 45 patients with non-bacterial pneumonia in the same period were selected as the control group.Serum CRP,PCT and IL-6 tests were performed in both groups.The serum CRP,PCT,and IL-6 levels in two groups of patients were compared.Using pathological tests as the gold standard,the efficacy of serum CRP,PCT and IL-6 single and combined test in early diagnosis of hospital acquired pneumonia was analyzed,including sensitivity,false-positive rate,false-negative rate and compliance rate.Results The observation group had serum CRP of(29.61±7.82)mg/L,PCT of(0.72±0.10)ng/ml,IL-6 of(26.42±3.01)pg/ml,which were higher than(9.75±1.36)mg/L,(0.18±0.05)ng/ml and(6.21±1.57)pg/ml in the control group(P<0.05).Serum CRP test showed 44 positive cases and 41 negative cases;serum PCT test showed 40 positive cases and 45 negative cases;serum IL-6 test showed 44 positive cases and 41 negative cases;serum CRP,PCT and IL-6 combined test showed 40 positive cases and 45 negative cases.The early diagnostic sensitivity,false-positive rate,false-negative rate,and compliance rate of serum CRP test for hospital-acquired pneumonia patients were 65.00%,40.00%,35.00%,and 63.25%;the early diagnostic sensitivity,false-positive rate,false-negative rate,and compliance rate of serum PCT test in patients with hospital-acquired pneumonia were 67.50%,28.89%,32.50%,and 69.41%;the early diagnostic sensitivity,false-positive rate,false-negative rate,and compliance rate of serum IL-6 test in patients with hospital-acquired pneumonia were 57.50%,46.67%,42.50%,and 55.29%;the early diagnostic sensitivity,false-positive rate,false-negative rate,and compliance rate of patients with hospital-acquired pneumonia by serum CRP,PCT,and IL-6 combined test were 87.50%,11.11%,12.50%,and 88.24% .The sensitivity and compliance rate of serum CRP,PCT,and IL-6 combined test in patients with hospital-acquired pneumonia were higher than those of single test of serum CRP,PCT,and IL-6,and the false-positive and false-negative rates were lower than those of single test of serum CRP,PCT,and IL-6(P<0.05).Conclusion Serum CRP,PCT,and IL-6 are highly expressed in patients with hospital-acquired pneumonia,and the combined test of the three indicators can be used for early auxiliary diagnosis of patients with hospital-acquired pneumonia,which can effectively improve the diagnostic accuracy of the disease and can be further vigorously promoted.
