简易视觉热量评估法在维持性血液透析患者膳食热量评估中的应用价值研究

目的 探讨简易视觉热量评估法应用于维持性血液透析(MHD)患者膳食热量评估中的可靠性和有效性。方法 选取在本院血液净化中心规则治疗的 100 例MHD患者为研究对象,行两部分研究。第一部分研究,由 2 名评估者使用自行设计的简易视觉热量评估法分别对 100 例患者进行 24 h摄入热量调查,探讨简易视觉热量评估法区间设置的可靠性;第二部分研究,由 2 名评估者分别使用简易视觉热量评估法和称重法调查 100 例患者 24 h热量摄入总量,检验两种方法的一致性。分析简易视觉热量评估法不同评分的实际能量摄入,以受试者工作特征曲线(ROC曲线)分析简易视觉热量评估法与称重法的一致性,比较简易视觉热量评估法与称重法用时。结果 第一部分结果显示,两位评估者一致性评价结果百分比一致性为 74%;Weighted Kappa系数为 0。755[95%CI=(0。660,0。849),P<0。001],说明简易视觉热量评估法区间设置可靠。第二部分结果显示,简易视觉热量评估法与称重法的一致性检验结果显示百分比一致性为 72%;Weighted Kappa系数为 0。767[95%CI=(0。682,0。853),P<0。001],说明简易视觉热量评估法与称重法具有较好的一致性;简易视觉热量评估法不同评分所对应的称重法的实际热量平均值均落在简易视觉热量评估法对应评分的总热量区间内;ROC曲线分析显示,以金标准称重法为参照,简易视觉热量评估法热量评估区间结果曲线下面积(AUC)为 0。704[95%CI=(0。590,0。818),P<0。002],灵敏性为 0。821,特异性为 0。528,约登指数为 0。349。表明在评估血液透析患者膳食总热量摄入方面,简易视觉热量评估法和称重法具有较好的一致性。简易视觉热量评估法评估用时(3。87±1。63)min,称重法评估用时(13。07±2。38)min,简易视觉热量评估法评估用时短于称重法,差异具有统计学意义(t=31。893,P<0。001)。结论 简易视觉热量评估法适用于MHD患者的膳食总热量评估。
Study on value of simple visual calorie estimation method in assessment of dietary caloric intake in maintenance hemodialysis patients
Objective To explore the reliability and effectiveness of simple visual calorie estimation method in assessment of dietary caloric intake in maintenance hemodialysis(MHD)patients.Methods A total of 100 cases of MHD patients treated in our hemodialysis center were selected for the two-part study.In the first part of the study,two assessors used the self-designed simple visual calorie estimation method to evaluate calorie intake within 24 h of the 100 patients,so as to confirm the reliability of interval setting of the simple visual calorie estimation method.In the second part of the study,24-h total calorie intake in the 100 patients was investigated by two assessors who used simple visual calorie estimation method and Food Weighted Record method respectively,so as to confirm the consistency of the two methods.The actual energy intake of patients with different scores of simple visual caloric estimation method was analyzed,and the consistency of simple visual caloric estimation method and Food Weighted Record method was analyzed by receiver operating characteristic(ROC)curve,and the time spending for simple visual caloric estimation method and Food Weighted Record method was compared.Results The first part of the study showed that the calorie results obtained by the two assessors were with a good consistency of 74%,and the Weighted Kappa coefficient was 0.755[95%CI=0.660,0.849),P<0.001],indicating that the interval setting of simplified visual caloric estimation method was reliable.The results of the second part showed that the consistency test between the simple visual calorie estimation method and the Food Weighted Record method showed a percentage consistency of 72%;the Weighted Kappa coefficient was 0.767[95%CI=(0.682,0.853),P<0.001],indicating that the simple visual calorie estimation method and the weighing method were in a good consistency.The actual caloric means of the weighing methods corresponding to the different scores of the simple visual calorie estimation method fell within the total caloric interval of the corresponding scores of the simple visual calorie estimation method.With a gold standard as Food Weighted Record method,the area under the ROC curve(AUC)of the simple visual calorie estimation method was 0.704[95%CI=(0.590,0.818),P<0.002],with the sensitivity of 0.821,the specificity of 0.528,the Youden index of 0.349,indicating that the simple visual calorie estimation method and the Food Weighted Record method were with a good consistency in evaluating the total dietary calorie intake of hemodialysis patients.The evaluation time of simple visual calorie estimation method was(3.87±1.63)min,and that of Food Weighted Record method was(13.07±2.38)min;the evaluation time of simple visual calorie estimation method was shorter than that of Food Weighted Record method,and the difference was statistically significant(t=31.893,P<0.001).Conclusion The simple visual calorie estimation method can be applied to the assessment of dietary caloric intake in MHD patients.
