Exploring the effectiveness of fibrobronchoscopic alveolar lavage in the treatment of severe pulmonary infections
Objective To explore the effectiveness of fibrobronchoscopic alveolar lavage in the treatment of severe pulmonary infections.Methods 76 patients with severe pulmonary infection were selected and divided into a control group and an observation group by random number table method,with 38 cases in each group.The control group was given conventional treatment,while the observation group was given fibrobronchoscopic alveolar lavage on the basis of conventional treatment.Both groups were compared in terms of treatment effect,arterial blood gas indicators[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),arterial oxygen saturation(SpO2)and pH],inflammatory indicators[interleukin-4(IL-4),interleukin-6(IL-6)and interferon-gamma(IFN-γ),recovery indicators(duration of symptom resolution,duration of mechanical ventilation,length of hospital stay).Results The total effective rate of 94.74%in the observation group was higher than 78.95%in the control group(P<0.05).After treatment,the observation group had PaO2 of(70.23±10.01)mm Hg(1 mm Hg=0.133 kPa),SpO2 of(94.35±5.01)%and pH of(7.38±0.04),which were higher than(64.16±8.05)mm Hg,(89.31±5.21)%and(7.35±0.03)in the control group;PaCO2 of(42.30±6.64)mm Hg in the observation group was lower than(56.23±4.68)mm Hg in the control group(P<0.05).After treatment,the observation group had IL-4 of(11.49±1.26)pg/ml,IL-6 of(7.95±2.55)pg/ml,and IFN-γ of(12.20±2.75)pg/ml,which were lower than(13.79±1.24),(10.35±2.93),and(18.36±3.79)pg/ml in the control group(P<0.05).The duration of symptom resolution,duration of mechanical ventilation,length of hospital stay were(8.65±3.38),(5.14±3.26),and(12.09±3.56)d in the observation group,which were shorter than(13.41±5.43),(9.62±5.17)and(15.31±5.88)d in the control group(P<0.05).Conclusion For severe pulmonary infection,fibrobronchoscopic alveolar lavage can improve clinical symptoms and blood gas indexes,reduce inflammation of patients,and accelerate recovery.
