Comparison on the therapeutic effect of different approaches for cholecystectomy in the treatment of gallstones with chronic cholecystitis
Objective To compare the therapeutic effect of different approaches for cholecystectomy in the treatment of gallstones with chronic cholecystitis.Methods 80 patients with gallstones and chronic cholecystitis were divided into a control group and an intervention group according to the different surgical approaches,with 40 cases in each group.Both groups underwent laparoscopic cholecystectomy,with approach of anterior cystohepatic triangle in the control group and approach of posterior cystohepatic triangle in the intervention group.The treatment time and recovery time,the levels of inflammatory factors[C-reactive protein(CRP)and interleukin-6(IL-6)]and the incidence of postoperative complications were compared between the two groups.Results There were no significant differences in operation time,intestinal function recovery time and hospital stay between the two groups(P>0.05).3 d after surgery,the levels of CRP and IL-6 in the two groups were significantly higher than those before surgery;the intervention group had CRP of(12.60±3.48)mg/L and IL-6 of(121.47±14.69)ng/L,which were lower than(17.35±3.72)mg/L and(155.06±15.52)ng/L in the control group(P<0.05).The incidence of postoperative complications in the intervention group was 5.00%,which was lower than 20.00% in the control group(P<0.05).Conclusion When performing cholecystectomy for gallstones with chronic cholecystitis,the approach of posterior cystohepatic triangle can alleviate inflammatory reactions and reduce complications,and it is recommended to be preferred.
