Efficacy of noninvasive ventilation combined with bovine pulmonary surfactant on neonatal respiratory distress syndrome and its influence on pulmonary oxygenation,blood gas index and pulmonary circulatory function
Objective To explore the efficacy of noninvasive ventilation combined with bovine pulmonary surfactant on neonatal respiratory distress syndrome(NRDS)and its influence on pulmonary oxygenation,blood gas index and pulmonary circulatory function.Methods 80 children with NRDS were selected as research subjects and divided into a control group(noninvasive ventilation treatment,n=40)and a study group(noninvasive ventilation combined with bovine pulmonary surfactant treatment,n=40)according to different treatment methods.Comparison was made on clinical efficacy,pulmonary oxygenation function[arterial oxygen saturation(SaO2),oxygenation index(PaO2/FiO2)],blood gas index[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)],pulmonary circulation function[pulmonary artery pressure(PAP),extravascular lung water(EVLW),pulmonary vascular resistance(PVR)between the two groups.Results After treatment,the total effective rate of 95.00% in the study group was higher than 80.00% in the control group(P<0.05).After treatment,SaO2,PaO2/FiO2 in both groups were higher than those before treatment;the study group had SaO2 of(95.37±3.52)% and PaO2/FiO2 of(295.66±30.35)mm Hg(1 mm Hg=0.133 kPa),which were higher than(91.62±3.43)% and(263.89±28.74)mm Hg in the control group(P<0.05).After treatment,PaO2 in both groups was higher than that before treatment,and PaCO2 was lower than that before treatment;PaO2 of(81.71±6.37)mm Hg in the study group was higher than(73.24±6.33)mm Hg in the control group,and PaCO2 of(36.42±4.79)mm Hg was lower than(43.34±5.27)mm Hg in the control group(P<0.05).After treatment,PAP,EVLW and PVR in both groups were lower than those before treatment;the study group had PAP of(22.47±1.51)mm Hg,EVLW of(5.56±1.62)ml/kg and PVR of(22.47±5.67)kPa·s/L,which were lower than(25.33±2.24)mm Hg,(8.41±2.55)ml/kg and(29.31±6.25)kPa·s/L in the control group(P<0.05).Conclusion For children with NRDS,non-invasive ventilation combined with bovine pulmonary surfactant has a significant effect,which can effectively improve the pulmonary oxygenation function,blood gas index and pulmonary circulation function of children.
