目的 探讨使用无创呼吸机联合有创机械通气治疗呼吸衰竭的疗效。方法 选取 60 例呼吸衰竭患者为研究对象,采用随机数字表法将患者分为对照组和观察组,每组 30 例。对照组患者执行的治疗方案为有创机械通气,观察组患者执行的治疗方案为无创呼吸机+有创机械通气。对比两组患者治疗效果、应激指标、心肺功能指标、呼吸机相关不良反应发生率。结果 观察组治疗总有效率96。67%高于对照组的 73。33%,差异有统计学意义(P<0。05);观察组患者心率(84。39±4。11)次/min、呼吸频率(21。14±1。96)次/min、动脉血二氧化碳分压(43。92±2。21)mm Hg(1 mm Hg=0。133 kPa)小于对照组的(113。02±5。43)次/min、(27。39±2。87)次/min、(58。29±3。98)mm Hg,动脉血氧饱和度(95。22±5。36)%、动脉血氧分压(95。82±6。43)mm Hg、pH值(7。49±0。13)大于对照组的(86。29±5。11)%、(86。99±5。43)mm Hg、(7。19±0。09),差异有统计学意义(P<0。05);观察组患者血管紧张素Ⅱ(31。29±3。11)pg/ml、肾素活性(2。21±0。34)ng/(ml·h)、去甲肾上腺素(131。02±15。54)ng/ml、肾上腺素(97。22±5。73)ng/ml小于对照组的(56。56±5。44)pg/ml、(4。19±0。76)ng/(ml·h)、(199。98±19。47)ng/ml、(158。43±9。87)ng/ml,差异有统计学意义(P<0。05);治疗后,观察组患者呼吸机相关不良反应发生率 3。33%低于对照组的 33。33%,差异有统计学意义(P<0。05)。结论 呼吸衰竭患者执行无创呼吸机+有创机械通气治疗疗效显著,患者的应激指标、心肺功能指标得到有效改善,呼吸机相关不良反应发生率低,临床应用价值显著。
Exploring the efficacy of noninvasive ventilator in combination with invasive mechanical ventilation in the treatment of respiratory failure
Objective To explore the efficacy of noninvasive ventilator in combination with invasive mechanical ventilation in the treatment of respiratory failure.Methods 60 patients with respiratory failure were selected as the study subjects,and the patients were divided into a control group and an observation group by random numerical table,with 30 cases in each group.The control group was treated with invasive mechanical ventilation,while the observation group was treated with noninvasive ventilator+invasive mechanical ventilation.The therapeutic effect,stress indicators,respiratory and indicators of cardio-pulmonary function,and the incidence of ventilator-related adverse reactions were compared between the two groups.Results The total effective rate of 96.67%in the observation group was higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).In the observation group,the heart rate was(84.39±4.11)beats/min,the respiratory rate was(21.14±1.96)times/min,and the arterial partial pressure of carbon dioxide was(43.92±2.21)mm Hg(1 mm Hg=0.133 kPa),which were lower than(113.02±5.43)beats/min,(27.39±2.87)times/min,and(58.29±3.98)mm Hg in the control group;the blood oxygen saturation was(95.22±5.36)%,the arterial partial pressure of oxygen was(95.82±6.43)mm Hg,and pH value was(7.49±0.13),which were higher than(86.29±5.11)%,(86.99±5.43)mm Hg,and(7.19±0.09)in the control group;the difference was statistically significant(P<0.05).In the observation group,the angiotensin Ⅱ was(31.29±3.11)pg/ml,the renin activity was(2.21±0.34)ng/(ml·h),the norepinephrine was(131.02±15.54)ng/ml,and the epinephrine(97.22±5.73)ng/ml,which were lower than(56.56±5.44)pg/ml,(4.19±0.76)ng/(ml·h),(199.98±19.47)ng/ml,and(158.43±9.87)ng/ml in the control group,and the difference was statistically significant(P<0.05).After treatment,the incidence of ventilator-related adverse reactions was 3.33%in the observation group,which was lower than 33.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of non-invasive ventilator+invasive mechanical ventilation in patients with respiratory failure is remarkable,and the patients'stress indicators,and indicators of cardio-pulmonary function have been effectively improved,and the incidence of ventilator-related adverse reactions is low,which is of significant clinical application value.
