Study the clinical effect of traditional anti-tuberculosis drugs combined with glucocorticoids in the treatment of tuberculous pleurisy
Objective To observe the feasibility of traditional anti-tuberculosis drugs combined with glucocorticoids in the treatment of tuberculous pleurisy,so as to provide useful reference for clinical research.Methods 70 patients with tuberculous pleurisy were selected as the study subjects,and were divided into a control group(34 cases)and an observation group(36 cases)according to lottery method.The control group was treated with traditional anti-tuberculosis drugs,and the observation group was treated with glucocorticoid based on the control group.The clinical indicators,levels of inflammatory factors in pleural effusion and lung function before and after treatment,and clinical effects of the two groups were compared.Results The total effective rate of 88.89%in the observation group was higher than 64.71%in the control group(P<0.05).After treatment,the thickness of pleural effusion in the two groups was lower than that before treatment in this group,and the thickness of pleural effusion of(1.12±0.26)mm in the observation group was lower than(1.35±0.29)mm in the control group(P<0.05).The drainage time of(7.38±1.01)d in the observation group was shorter than(11.65±1.30)d in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α,interleukin-2,and interleukin-17 in pleural effusion of the control group were(158.39±17.10),(311.27±23.15),and(42.27±4.19)pg/ml,and those of the observation group were(117.86±13.72),(286.51±21.90),and(35.96±3.05)pg/ml.After treatment,the levels of TNF-α,interleukin-2 and interleukin-17 in pleural effusion of the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).After treatment,the ratio of forced expiratory volume in one second to forced vital capacity(FEV1/FVC),forced expiratory volume in one second(FEV1)as a percentage of the predicted value,and maximal inspiratory pressure of the control group were(67.54±8.30)%,(79.81±7.75)%,and(95.14±18.65)cm H2O(1 cm H2O=0.098 kPa),while those of the observation group were(71.19±5.05)%,(83.70±6.42)%,and(106.83±19.11)cm H2O.After treatment,FEV1/FVC,FEV1 as a percentage of the predicted value,and maximal inspiratory pressure in both groups were higher than those before treatment,and the observation group was higher than the control group(P<0.05).Conclusion Traditional anti-tuberculosis drugs combined with glucocorticoids for patients with tuberculous pleurisy has a more satisfactory effect,which can shorten the drainage time of patients,reduce the thickness of pleural effusion,reduce the level of inflammation,and then promote the early recovery of lung function.
