Analysis of risk factors for venous thrombosis after abdominal surgery in elderly patients
Objective To analyze the risk factors for venous thrombosis after abdominal surgery(AS)in elderly patients.Methods The clinical data of 138 elderly patients who underwent abdominal surgery were retrospectively analyzed,and based on the postoperative clinical examination results,patients who developed venous thrombosis were classified into the occurrence group(n=35),and those who did not develop venous thrombosis were classified into the non-occurrence group(n=103).The general data and clinical characteristics of the two groups of patients were collected and compared.For indicators with differences,binary Logistic regression was used to analyze the risk factors for venous thrombosis in elderly patients after abdominal surgery,and the receiver operating characteristic(ROC)curves were plotted to calculate the sensitivity,specificity and the area under the curve(AUC),and the predictive value of each factor to the occurrence of postoperative venous thrombosis was observed.Results There was no significant difference in the comparison of gender,history of alcohol consumption,family history of disease,per capita monthly income,mode of surgery,grade of anesthesia,history of hypertension,history of diabetes mellitus,pulmonary infections,and previous venous thrombosis between the two groups(P>0.05).In the occurrence group,62.86%of patients were>80 years old,60.00%had a history of smoking,65.71%had postoperative bedridden time≥72 h,68.57%had limb movement limitation,74.29%had postoperative fear of exercise,57.14%had a body mass index(BMI)of≥24 kg/m2,and 62.86%had an operation time of>30 min,which were higher than 33.98%,32.04%,38.83%,37.86%,48.54%,31.07%,and 35.92%in the non-occurrence group,and there were significant differences(P<0.05).>80 years of age,postoperative bedridden time≥72 h,limb movement limitation,postoperative fear of exercise,history of smoking,BMI≥24 kg/m2,and operation time>30 min were all independent risk factors for venous thrombosis after abdominal surgery in elderly patients(all OR>1,P<0.05).The AUCs for age,postoperative bedridden time,limb movement limitation,postoperative fear of exercise,history of smoking,BMI and operation time were 0.644,0.634,0.649,0.629,0.640,0.630 and 0.635,the sensitivities were 0.629,0.657,0.686,0.743,0.600,0.571 and 0.629,and the specificities were 0.660,0.612,0.612,0.515,0.680,0.689 and 0.641.It indicated that based on>80 years of age,postoperative bedridden time≥72 h,limb movement limitation,postoperative fear of exercise,history of smoking,BMI≥24 kg/m2 and operation time>30 min,there were 64.4%,63.4%,64.9%,62.9%,64.0%,63.0%and 63.5%chances of predicting the occurrence of venous thrombosis after abdominal surgery.Conclusion Risk factors for venous thrombosis after abdominal surgery in elderly patients include>80 years of age,postoperative bedridden time≥72 h,limb movement limitation,postoperative fear of exercise,history of smoking,BMI≥24 kg/m2,and operation time>30 min,and each factor has a high predictive value for the occurrence of postoperative venous thrombosis in patients.
