Application value analysis of chemiluminescence immunoassay in biochemical immunoassay of thyroid function
Objective To explore the application value of chemiluminescence immunoassay in biochemical immunoassay of thyroid function.Methods 120 suspected patients with thyroid dysfunction were selected for biochemical immunoassay of thyroid function.Fasting peripheral venous blood from patients were collected for biochemical immunoassay,radioimmunoassay and chemiluminescence immunoassay.Based on the clinical comprehensive diagnosis results,the diagnostic efficacy of radioimmunoassay and chemiluminescence immunoassay for thyroid dysfunction were compared,and the consistency between the two biochemical immunoassay and the clinical comprehensive diagnosis was analyzed.Results Among 120 patients with suspected thyroid dysfunction,31 cases of hyperthyroidism were diagnosed after comprehensive clinical diagnosis,and the remaining 89 patients did not have hyperthyroidism.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of chemiluminescence immunoassay for the diagnosis of hyperthyroidism were 96.77%,96.63%,96.67%,90.91%,and 98.85%,which were higher than 77.42%,86.52%,84.17%,66.67%,and 91.67%of radioimmunoassay(P<0.05).After consistency analysis,the results of chemiluminescence immunoassay showed a high degree of consistency with the clinical comprehensive diagnosis(Kappa value=0.895);while the results of radioimmunoassay showed only moderate consistency with the clinical comprehensive diagnosis(Kappa value=0.631).Among 120 patients with suspected thyroid dysfunction,65 cases of hypothyroidism were diagnosed after comprehensive clinical diagnosis,and the remaining 55 patients did not have hypothyroidism.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of chemiluminescence immunoassay for the diagnosis of hypothyroidism were 96.92%,98.18%,97.50%,98.44%and 96.43%,which were higher than 86.15%,85.45%,85.83%,87.50%and 83.93%of radioimmunoassay(P<0.05).After consistency analysis,the results of chemiluminescence immunoassay showed a high degree of consistency with the clinical comprehensive diagnosis(Kappa value=0.876);while the results of radioimmunoassay showed only moderate consistency with the clinical comprehensive diagnosis(Kappa value=0.647).Conclusion In the biochemical immunoassay of thyroid function,the application of chemiluminescence immunoassay can provide sensitive and accurate diagnosis of thyroid dysfunction such as hyperthyroidism and hypothyroidism,and its diagnostic value is higher than that of radioimmunoassay.
