Observation on the preventive effect of acupoint application combined with acupoint massage on postoperative nausea and vomiting after total hip arthroplasty under the concept of fast-track surgery
Objective To explore the preventive effect of acupoint application combined with acupoint massage on postoperative nausea and vomiting(PONV)after total hip arthroplasty under the concept of fast-track surgery.Methods 100 patients who underwent total hip arthroplasty were divided into two groups according to the order of admission,in which 50 odd-numbered patients were selected as the control group,and 50 even-numbered patients were selected as the experimental group.The patients in the control group were given routine postoperative care under the concept of fast-track surgery,and the patients in the experimental group were given acupoint application combined with acupoint massage on the basis of the control group.The occurrence of PONV,comfort level,postoperative clinical rehabilitation indexes and adverse events were compared between the two groups.Results The occurrence of PONV in the experimental group at 0-5,6-11,24-48 h after surgery and in total was significantly better than that in the control group,and there were significant differences(Z=3.013,2.041,2.282,5.365;P<0.05).There was no significant difference in occurrence of PONV between the two groups at 12-23 h after surgery(Z=1.495,P>0.05).The incidence of PONV in the experimental group was 18.0%(9/50),which was significantly lower than 74.0%(37/50)in the control group,and there was a significant difference(χ2=31.562,P<0.05).The duration of PONV of 9 patients in the experimental group was(3.64±1.39)h and that of 37 patients in the control group was(6.07±1.82)h.The duration of PONV of the experimental group was shorter than that of the control group,and there was a significant difference(t=3.737,P<0.05).The General Comfort Questionnaire(GCQ)scores of the experimental group at 6,12 and 48 h after surgery were(75.41±4.98),(82.29±9.34)and(87.15±6.97)points,which were higher than(72.68±6.32),(78.06±10.67)and(83.27±9.04)points of the control group,and there were significant differences(t=2.399,2.109,2.403;P<0.05).The ambulation time and hospitalization time of the experimental group were(26.46±9.54)h and(11.84±3.95)d,which were shorter than(32.65±11.09)h and(13.66±4.37)d of the control group,and there were significant differences(t=2.992,2.185;P<0.05).There was no significant difference in the incidence of adverse events between the two groups(χ2=2.041,P>0.05).Conclusion Postoperative acupoint application combined with acupoint massage is safe and effective in preventing PONV after total hip arthroplasty,and can reduce the incidence of PONV,improve the comfort of patients and speed up the rehabilitation process of patients.
