Effect of oral nutritional supplement on hematological complications and infections of children with acute lymphoblastic leukemia in the stage of induction chemotherapy—middle term results of a randomized controlled study
Objective To explore the effect of oral nutritional supplement(ONS)intervention on hematological complications and infections of children with acute lymphoblastic leukemia(ALL)in the stage of induction chemotherapy.Methods Totally 36 children with ALL younger than 16 years old who met the inclusion criteria and completed induction chemotherapy in our hospital from January 2022 to December 2022 were enrolled for a prospective randomized controlled trial.A random number table was used to randomly divide the enrolled children into the intervention group and the control group at a ratio of 1∶1 with 18 cases in each group.The control group was given low-fat diet(dietary fat energy ≤20%of total daily energy),and the intervention group was given low-fat diet and short peptide type ONS with high medium-chain triglycerides,high protein and high calorie and a dose of 20mL/(kg·d).The consumption time was during the stage of VDLD induction chemotherapy.Hematological complications,the transfusion of red blood cells and platelets and infections were observed in the stage of induction chemotherapy.Results Compared with the control group,the duration of neutropenia was shorter(12.50±9.38 days vs 19.11±10.0 days,P=0.049),and the recovery time of neutropenia was significantly earlier(25.39±4.5 days vs 30.06±3.80 days,P=0.002)in the intervention group.The lowest platelet count was increased in the intervention group(P=0.048).In terms of Ⅳ degree of thrombocytopenia,its occurrence time was later(P=0.009),the duration days was shorter(P=0.024)and the recovery time was earlier(P=0.012)than the control group.The number and times of receiving platelet transfusion in the intervention group were lower in the intervention group than that in the control group(P<0.05).Days of fever,types and days of antibiotic use,incidence of infection events tended to be lower in the intervention group,but the difference was no statistically significant(P>0.05).Conclusions At the stage of induction chemotherapy,ONS with high medium-chain triglycerides,high protein and high calorie can improve the tolerance of hematologic system to chemotherapy.Definitive conclusions will be drawn with the results of the final study.
