摘要
目的 探讨经导管心脏射频消融术后尿酸/白蛋白比值(UAR)与心房颤动(房颤)复发之间的关系.方法 选取潍坊市人民医院2022年1月至12月首次行经导管射频消融术的232例房颤患者进行回顾性研究,门诊随访12个月根据有无复发分为复发组和正常组.比较两组的临床、实验室特征,通过logistic回归分析评估复发因素,以受试者工作特征曲线(ROC)分析UAR对复发的预测价值.结果 54例(23.3％)患者在经导管消融后出现房颤复发.复发组的平均UAR(1.57±0.39)高于正常组(1.34±0.35).多因素logistics回归分析显示,左心房内径(OR=3.649,95％CI 1.041～1.225,P＜0.01)和UAR(OR=1.129,95％CI 1.392～9.566,P＜0.01)是复发的独立预测因素.在ROC分析中,UAR预测房颤射频消融术后复发的曲线下面积(AUC)为0.663(95％CI0.58～0.74),敏感度为75.9％,特异度为51.1％.结论 房颤经导管心脏射频消融术后复发与UAR存在独立相关性.
Abstract
Objective To explore the association between the uric acid/albumin ratio(UAR)and the recurrence of atrial fibrillation following transcatheter cardiac radiofrequency ablation.Methods A total of 232 patients with atrial fibrillation who underwent radiofrequency catheter ablation for the first time from January 2022 to December 2022 in Weifang People's Hospital were included.The patients were followed up for 12 months and divided into the recurrence group and normal group according to the presence or absence of recurrence.The clinical and laboratory characteristics of the two groups were compared.The predictive factors of recurrence were evaluated by logistic regression analysis.The predictive value of UAR for the recurrence was analyzed by the receiver operating characteristic curve(ROC).Results Recurrence of atrial fibrillation occurred in 54 patients(23.0％)after the catheter ablation.The mean UAR of the recurrent group(1.57±0.39)was higher than that of the normal group(1.34±0.35).Multivariate logistic regression analysis showed that left atrial diameter(OR=3.649,95％CI 1.041-1.225,P＜0.01)and UAR(OR=1.129,95％CI 1.392-9.566,P＜0.01)were the independent predictors of recurrence.In the ROC analysis,the area under the AUC of UAR in predicting the recurrence of atrial fibrillation after the radiofrequency ablation was 0.663(95％CI 0.58-0.74),the sensitivity was 75.9％,and the specificity was 51.1％.Conclusion The recurrence of atrial fibrillation after radiofrequency ablation is independently correlated with UAR.
