血清脂肪酸结合蛋白4及磷脂酰肌醇3-激酶与心力衰竭的相关性研究

摘要
目的 探讨血清脂肪酸结合蛋白4(FABP4)及磷脂酰肌醇3-激酶(PI3K)的水平变化与心力衰竭发生、发展的关系.方法 选取2023年10月至2024年4月安徽理工大学第一附属医院心内科住院心力衰竭患者89例,其中心功能Ⅲ级组41例,心功能Ⅳ级组48例,以及同时期入院检查排除心力衰竭患者89例作为对照组.采用ELISA法检测各组受试者血清FABP4、PI3K水平,分析两者在不同分组间的差异及临床意义,并应用Pearson相关性进一步检验FABP4、PI3K与N末端-B型钠尿肽前体(NT-proBNP)相关性.结果 与对照组相比,心功能Ⅲ级组及心功能Ⅳ级组患者的年龄、左心房前后径(LAD)、左心室舒张末期内径(LVDD)、左心室收缩末期内径(LVSD)、N末端-B型钠尿肽前体(NT-proBNP)、肌酐(Cr)、C反应蛋白(CRP)、空腹血糖(FPG)水平显著升高,而甘油三酯(TG)、左心室射血分数(LVEF)、左心缩短分数(FS)水平降低,差异均具有统计学意义(P<0.05);心功能Ⅲ级组及心功能Ⅳ级组血清FABP4表达水平升高[(64.32±21.58)ng/ml 比(75.20±26.37)ng/ml 比(54.16±15.45)ng/ml.P<0.05]、血清 PI3K 表达水平下降[(889.07±155.23)U/L 比(763.53±184.60)U/L 比(980.37±201.86)U/L,P<0.05].与心功能Ⅲ级相比,心功能Ⅳ级组血清FABP4 表达水平升高[(75.20±26.37)ng/ml 比(64.32±21.58)ng/ml,P<0.05]、血清 PI3K 表达水平下降[(763.53±184.60)U/L 比(889.07±155.23)U/L,P<0.05].Pearson法对FABP4、PI3K与NT-proBNP进行相关性分析,结果显示在所有实验组中FABP4水平与 NT-proBNP大小呈正相关关系(r=0.711,P<0.05);PI3K与NT-proBNP 呈负相关关系(r=-0.803,P<0.05),FABP4 与 PI3K 水平呈负相关关系(r=-0.434,P<0.05).结论 HF患者血清中FABP4表达水平明显升高,并随着心功能分级增加而逐渐增加,PI3K表达水平明显降低,并随着心功能分级增加而逐渐降低,二者都与NT-proBNP有一定的相关性.可见,FABP4、PI3K的激活与表达与HF的发生可能有关.
Abstract
Objective To investigate the relationship between the levels of serum fatty acid binding protein 4(FABP4)and phosphatidylinositol 3-kinase(PI3K)and the occurrence and development of heart failure(HF).Methods 89 patients with heart failure hospitalized in the Department of Cardiology of the First Affiliated Hospital of Anhui University of Science and Technology from October 2023 to April 2024 were selected,of which 41 were in the group of cardiac function class Ⅲ and 48 were in the group of cardiac function class Ⅳ,as well as 89 patients who were admitted to the hospital for the examination and exclusion of heart failure in the same period as the control group.The serum levels of FABP4 and PI3K in each group were measured by ELISA to observe the differences and clinical significance of the two between different subgroups;and Pearson correlation analysis was applied to further analyze the correlation between both FABP4 and PI3K and N-terminal-B-type natriuretic peptide precursor(NT-proBNP).Results The age,left atrial anteroposterior diameter(LAD),left ventricular end-diastolic internal diameter(LVDD),left ventricular end-systolic internal diameter(LVSD),N-terminal-B-type natriuretic peptide precursor(NT-proBNP),creatinine(Cr),C-reactive protein(CRP),and fasting glucose(FPG)levels of the patients in the cardiac function class Ⅲ and class Ⅳ groups were higher than that in the control group,whereas the levels of triglycerides(TG),left ventricular ejection fraction(LVEF)and left shortening fraction(FS)levels were lower than those in the control group(P<0.05);serum FABP4 expression levels were higher in the cardiac function class Ⅲ and cardiac function class Ⅳ groups than those in the control group[(64.32±21.58)ng/ml vs.(75.20±26.37)ng/ml vs.(54.16±15.45)ng/ml,P<0.05],but serum PI3K expression level was lower than that of the control group[(889.07±1 55.23)U/L vs.(763.53±184.60)U/L vs.(980.37±201.86)U/L,P<0.05].Compared with the cardiac function class Ⅲ group,serum FABP4 expression level was increased in the cardiac function class Ⅳ group[(75.20±26.37)ng/ml vs.(64.32±21.58)ng/ml,P<0.05],serum PI3K expression level was decreased[(763.53±184.60)U/L vs.(889.07±155.23)U/L,P<0.05].The Pearson correlation analysis showed that FABP4 level was positively correlated with the level of NT-proBNP(r=0.711,P<0.05),but PI3K was negatively correlated with NT-proBNP(r=-0.803,P<0.05),and FABP4 was negatively correlated with the level of PI3K(r=-0.434,P<0.05)in all the experimental groups.Conclusion The expression level of FABP4 in the serum of HF patients is significantly higher and gradually increase with the raise of the cardiac function grading,but the expression level of PI3K appears just opposite to FABP4,and both they have a certain correlation with NT-proBNP.
