甘油三酯葡萄糖指数和超声心动图指标与心力衰竭的相关性研究

摘要
目的 评估心力衰竭(HF)中甘油三酯葡萄糖(TyG)指数及超声心动图指标变化,分析HF相关危险因素,探究TyG指数与HF之间的剂量-反应关系.方法 纳入2020年1月至2022年12月在新疆医科大学第五附属医院心血管内科住院的149例HF患者(HF组)和89例非HF患者(非HF组).分析各组临床资料、实验室指标及超声心动图指标,计算TyG指数.采用二元logistic回归分析得出HF的危险因素,使用受试者工作特征曲线(ROC)下面积评估TyG指数和超声心动图指标对HF的预测价值,构建限制性立方样条(RCS)模型明确TyG指数与HF之间的剂量-反应关系.结果 Logistic回归显示,左心室舒张末期内径(LVEDD)、右心室内径(RVD)、右心房内径(RAD)、左心房内径(LAD)是HF的危险因素,OR分别为1.147(95％CI 1.093～1.204)、1.491(95％CI 1.309～1.698)、1.292(95％CI 1.193～1.400)、1.431(95％CI 1.303～1.571),缩短分数(FS)、TyG 指数是 HF 的保护因素,OR分别为 0.756(95％C/0.699～0.817)、0.175(95％CI 0.087～0.351).RCS模型结果显示,TyG指数与HF之间存在剂量-反应关系(P非线性=0.521,P整体=0.008).结论 低水平TyG指数(TyG≤9)为HF的保护因素,对HF的预测具有一定的价值,两者之间存在线性剂量-反应关系.
Abstract
Objective To assess the changes of triglyceride glucose(TyG)index and echocardiographic indexes in heart failure(HF),analyse the risk factors related to HF,and explore the dose-response relationship between TyG index and HF.Methods A total of 149 HF patients(HF group)and 89 non-HF patients(non-HF group)who were hospitalized in the Department of Cardiovascular Medicine of the Fifth Affiliated Hospital of Xinjiang Medical University from January 2020 to December 2022.The clinical data,laboratory indexes and echocardiographic indexes were analyzed,and the TyG index was calculated.Binary logistic regression analysis was used to derive the risk factors for HF;the predictive value of TyG index and echocardiographic indexes for HF were evaluated using the receicer operating characteristic(ROC)curve,and the restricted cubic spline(RCS)model was constructed to clarify the dose-response relationship between the TyG index and HF.Results Logistic regression showed that left ventricular end-diastolic diameter(LVEDD),right ventricular diameter(RVD),right atrial diameter(RAD)and left atrial diameter(LAD)were the risk factors for HF,with ORs of 1.147(95％CI 1.093-1.204),1.491(95％CI 1.309-1.698),1.292(95％CI 1.193-1.400),1.431(95％CI 1.303-1.571),but the fractional shortening(FS)and TyG index were the protective factors for HF with OR 0.756(95％CI 0.699-0.817)and 0.175(95％CI 0.087-0.351).The results of the RCS model showed a dose-response relationship between the TyG index and HF(Pnon-linear=0.521,Poverall=0.008).Conclusion Low level of TyG index(TyG≤9)would be a protective factor for HF and have some value in the prediction of HF,with a linear-dose-response relationship between them.
