慢性心力衰竭患者心功能与心肌纤维化指标的相关性研究

目的 观察慢性心力衰竭(CHF)患者心功能与心肌纤维化指标,分析心功能与心肌纤维化指标的关系.方法 选择2021-2022年本院收治的173例CHF患者,根据纽约心脏病协会(NYHA)心功能分级将CHF患者分为NYHA Ⅲ级组、NYHA Ⅳ级组,比较2组的心功能与心肌纤维化指标,分析CHF患者心功能与心肌纤维化指标的相关性.结果 NYHA Ⅳ级组左心室收缩末期内径(LVESD)为(42.63±5.47)mm、左心室舒张末期内径(LVEDD)为(61.24±9.26)mm,高于 NYHA Ⅲ级组的(37.55±4.55)mm、(50.72±8.60)mm;左室射血分数(LVEF)为(33.18±3.41)%,低于 NYHA Ⅲ 级组的(39.18±3.66)%,差异均有统计学意义(t=6.682、7.747、11.086,P均<0.001).NYHA Ⅳ级组半乳糖凝集素-3(Galectin-3)为(18.74±4.13)ng/mL、可溶性生长刺激表达基因2蛋白(sST2)为(55.25±9.01)ng/mL、Ⅲ型前胶原(PCⅢ)为(121.76±14.45)μg/L,高于NYHA Ⅲ级组的(15.91±3.07)ng/mL、(47.82±7.50)ng/mL、(106.43±16.02)μg/L,差异均有统计学意义(t=5.191、5.916、6.578,P均<0.001).Pearson相关性分析显示:CHF患者LVESD、LVEDD与Galectin-3、sST2、PC Ⅲ呈正相关(r>0,P<0.001);LVEF与Galectin-3、sST2、PCⅢ呈负相关(r<0,P<0.001).绘制ROC曲线发现,Galectin-3、sST2、PC Ⅲ评估CHF患者心功能的AUC分别为0.700、0.735、0.769.结论 心肌纤维化指标Galectin-3、sST2、PC Ⅲ与CHF患者心功能恶化有关,可在一定程度上评估心功能.
Analysis of the correlation between cardiac function and myocardial fi-brosis indicators in patients with chronic heart failure
Objective To observe heart function and myocardial fibrosis indicators in patients with chronic heart failure(CHF),and to analyze the relationship between heart function and myocardial fibrosis indicators.Methods 173 pa-tients with CHF who were admitted to hospital in 2021 or 2022 were selected.According to the New York Heart Association(NYHA)cardiac function classification,patients with CHF were divided into the NYHA Ⅲ group and NYHA Ⅳ group.The cardiac function and myocardial fibrosis indicators of the two groups were compared,and the correlation between cardiac function and myocardial fibrosis indicators in patients with CHF was analyzed.Results The left ventricular end systolic di-ameter(LVESD)[(42.63±5.47)mm]and left ventricular end diastolic diameter(LVEDD)[(61.24±9.26)mm]in the NYHA grade Ⅳ group were higher than those in the NYHA grade Ⅲ group[(37.55±4.55)mm and(50.72±8.60)mm,respective-ly].The left ventricular ejection fraction(LVEF)[(33.18±3.41)%]was lower than that in the NYHA grade Ⅲ group[(39.18±3.66)%],and the differences were statistically significant(t=6.682,7.747,11.086,all P<0.001).In the NYHA grade Ⅳgroup,the concentration of Galactin-3 was(18.74±4.13)ng/mL,that of soluble growth stimulating gene 2 protein(sST2)was(55.25±9.01)ng/mL,and type Ⅲ procollagen(PC Ⅲ)(121.76±14.45)µg/L,and these were higher than those in the NYHA grade Ⅲ group[(15.91±3.07)ng/mL,(47.82±7.50)ng/mL,and(106.43±16.02)μg/L];the differences were statisti-cally significant(t=5.191,5.916,6.578,all P<0.001).Pearson correlation analysis showed that the LVESD and LVEDD in patients with CHF were positively correlated with Galectin-3,sST2,and PC Ⅲ(r>0,P<0.001).There was a negative correla-tion between LVEF and Galectin-3,sST2,and PC Ⅲ in these patients(r<0,P<0.001).The ROC curve showed that the AUC for Galectin-3,sST2,and PC Ⅲ,when used for as-sessing cardiac function in patients with CHF,were 0.700,0.735,and 0.769.Conclusion The indicators of myocardial fibrosis,Galectin-3,sST2,and PC Ⅲ,are related to the deteri-oration of cardiac function in patients with CHF,and can be used to evaluate cardiac function to a certain extent.
