远山脉冲电场消融系统治疗心房颤动初步疗效观察

摘要
目的 研究国产远山脉冲电场消融(PFA)治疗心房颤动(简称房颤)的近期效果和安全性.方法 入选2022年6月至2023年1月住院接受国产PFA系统消融治疗的房颤患者18例,观察术后即刻的肺静脉隔离情况,围手术期手术并发症及3个月随访结果.结果 18例患者中阵发性房颤16例,持续性房颤2例,2例持续性房颤患者除完成了肺静脉隔离外,在顶部线和后壁增加消融,所有患者均实现肺静脉隔离(100％).术后住院期间复发2例,均未给予处理自行转为窦性心律,左上肺消融过程中出现心率下降2例,停止消融后很快恢复,围手术期所有患者均未出现相关并发症.术后3个月随访,有2例24 h动态心电图超过30 s的房颤发作.结论 初步研究显示国产远山PFA系统消融治疗房颤是可行和安全的.
Abstract
Objective To study the short-term clinical efficacy and safety of YuanShan pulsed field ablation(PFA)system in the treatment of atrial fibrillation(AF).Method Eighteen patients with AF(16 paroxysmal,2 persis-tent)from June 2022 to January 2023 were underwent PFA treatment were selected.Immediate postoperative pulmo-nary vein isolation and perioperative surgical complications were observed.Result All patients achieved pulmona-ry vein ablation,ablation was added to the top line and posterior wall for 2 persistent patients.All patients achieved pulmonary vein isolation(100％).There were 2 patients recurrence during the postoperative hospitalization,none treatment the spontaneously conversion to sinus rhythm.During the left superior pulmonary vein ablation process,there were 2 cases with decreased heart rate,after stopping ablation,they quickly recovered.None of the patients had happened complications during the perioperative period.Conclusion Preliminary studies have shown that the YuanShan PFA system for ablation of AF is clinically feasible and safe.[Chinese Journal of Cardiac Pacing and E-lectrophysiology,2024,38(1):30-33]
