目的 探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与室性心律失常的相关性。方法 选取2021年1月至2023年9月经复旦大学附属中山医院厦门医院睡眠中心初筛为OSAHS患者为研究对象,完成基本资料收集、多导睡眠图(PSG)及动态心电图监测,采用Lown分级系统对室性心律失常进行分级。根据呼吸暂停低通气指数(AHI)将OSAHS患者病情程度分为轻度、中度、重度组,比较各组一般临床特征和全天、清醒期及睡眠期室性心律失常发生情况;进一步对睡眠期Lown分级是否Ⅲ级及以上分组,单因素分析组间一般情况及其与PSG参数的相关性,二分类logistic回归分析室性心律失常的影响因素。结果 共入选199例,其中轻、中、重度组分别为63、65、71例。随着OSAHS病情程度增加,体质量指数(BM1)、睡眠期Lown分级Ⅲ级及以上患者比例呈上升趋势(P<0。05);与睡眠期Lown分级Ⅲ级以下患者相比,睡眠期Lown分级Ⅲ级及以上患者年龄、冠心病比例、夜间脉搏血氧饱和度(SpO2)<90%的睡眠时间占比(T90)均增加,平均SpO2、最低SpO2均降低(P<0。05);二分类logistic回归分析发现睡眠期Lown分级Ⅲ级及以上患者较Ⅲ级以下患者年龄增加[比值比OR=1。050,95%可信区间(CI)=1。017~1。085],而最低SpO2降低(OR=0。926,95%CI=0。865~0。991,P均<0。05)。结论 OSAHS 病情程度越重,睡眠期 Lown 分级 Ⅲ 级及以上患者比例越高,年龄、最低SpO2是OSAHS患者合并睡眠期Lown分级Ⅲ级及以上的独立相关因素。
Correlation between obstructive sleep apnea hypopnea syndrome and ventricular arrhythmia
Objective To explore the relationship between obstructive sleep apnea hypopnea syndrome(OSAHS)and ventricular arrhythmia.Methods Patients with OSAHS admitted to Zhongshan Hospital,Fudan University(Xiamen Branch)from January 2021 to September 2023 were using the Lown system to classify the ventricular ar-rhythmia.All subjects completed the general data collection,polysomnography(PSG)and Holter.According to the apnea hypopnea index(AHI),the severity of OSAHS patients was divided into mild group,moderate group,and severe group.The general clinical characteristics and incidence of ventricular arrhythmias during the whole day,a-wake and sleep periods were compared among each group;Further grouping was conducted to determine whether Lown level Ⅲ or above during sleeping.One-way analysis of variance was used to compare the general clinical char-acteristics and PSG parameters among different groups.Binary logistic regression analysis was conducted to investi-gate the influencing factors of ventricular arrhythmias.Results A total of 199 patients was divided into mild group(n=63),moderate group(n=65)and severe group(n=71).The results showed that as the severity of OSAHS increased,the proportion of patients with body mass index(BMI)and Lown level Ⅲ or above during sleeping were increased(P<0.05);Compared with patients with Lown level Ⅲ or below,patients with Lown level Ⅲ or above during sleeping have an increase in age,proportion of coro-nary heart disease and the percentage of cumulative time with oxygen saturation below 90%in total sleep time(T90),while average pulse oxygen saturation(SpO2)and lowest SpO2 were decreased(P<0.05).The binary logistic regression analysis found that patients with Lown levelⅢ or above during sleeping increased in age[odds ratio(OR)=1.050,95%confidence interval(CI)=1.017-1.085]and decreased in minimum SpO2(OR=0.926,95%CI=0.865-0.991)compared to patients with Lown level Ⅲ or below(all P<0.05).Conclusion The more severe the condition of OSAHS,the higher the proportion of patients with Lown level Ⅲ or above during sleeping.Age and the lowest SpO2 are independent related factors for OSAHS patients with sleep stage Low grade Ⅲ and above.[Chinese Journal of Cardiac Pacing and Electrophysiology,2024,38(2):105-108]
