日间手术模式下行经皮导管射频消融术的可行性及安全性

摘要
目的 探讨日间手术模式下行经皮导管射频消融术治疗心律失常的可行性及安全性.方法 连续性纳入2023年7月至2023年12月于宁波大学附属第一医院心内科住院且在日间手术模式下行经皮导管射频消融术的心律失常患者,出院后1周及1个月进行随访,记录围术期及随访期间不良事件发生情况.结果 共入选253例患者,其中男115例(45.5％),平均年龄(46.3±13.6)岁,室上性心动过速186例,室性早搏67例.所有患者均成功接受导管射频消融术,其中252例(99.6％)完成日间手术流程.7例(2.8％)出现穿刺部位瘀斑,延长加压包扎时间,瘀斑未进一步扩大,术后次日予出院.1例(0.4％)血管超声提示动静脉瘘,延长加压包扎后瘘口闭合.围术期未出现假性动脉瘤、心包积液及心包填塞等并发症.出院后1个月门诊随访,发生围术期并发症的患者已痊愈,其余患者未出现新发的并发症.结论 日间手术模式下行经皮导管射频消融术治疗心律失常可能是安全、可行的.
Abstract
Objective To investigate the feasibility and safety of ambulatory percutaneous radiofrequency catheter ablation(RFCA)in the treatment of arrhythmia.Methods Patients with arrhythmia and candidate for ambulatory RFCA was consecutively included in the Department of Cardiology,the First Affiliated Hospital of Ningbo Universi-ty from July 2023 to December 2023.Follow-up visits were arranged 1 week and 1 month after discharge.Complica-tions were recorded periprocedurally and during follow-up.Results A total of 253 patients(45.5％male,aged 46.3±13.6 years)were selected.The numbers of supraventricular tachycardia,and ventricular premature were 186 and 67,respectively.All the patients were successfully undergoing RFCA,among which 252 patients(99.6％)finished the ambulatory operation process.Seven patients had ecchymosis at site of puncture.The duration of compression bandage was prolonged.Arteriovenous fistula was detected in one patient by ultrasound.The fistula was closed after prolonging the application of compression bandage.No pseudoaneurysm,pericardial effusion and pericardial tampon-ade occurred periprocedurally.Patients with periprocedural complications all recovered at 1-month follow-up.No newly developed adverse events occurred.Conclusion Ambulatory RFCA in the treatment of arrhythmia may be safe and feasible.[Chinese Journal of Cardiac Pacing and Electrophysiology,2024,38(5):326-328]
