超声心动图与核磁共振成像对冠心病合并心力衰竭患者心功能的评估比较

摘要
目的 比较并分析超声心动图与核磁共振成像(MRI)对冠状动脉粥样硬化性心脏病(冠心病)合并心力衰竭(心衰)患者心功能的评估效果.方法 选取2020年7月至2023年5月于首都医科大学附属北京潞河医院心内科收治的80例冠心病合并心衰患者为研究对象,所有患者均行超声心动图、MRI检查,比较两种检查方法的左心功能参数[舒张末期容积(EDV)、收缩末期容积(ESV)、每搏输出量(SV)、射血分数(EF)].参照纽约心脏病协会(NYHA)分级标准,将患者分为Ⅰ～Ⅱ级(A组)、Ⅲ～Ⅳ级(B组),比较两组的超声心动图参数、MRI参数.根据心衰特点,将患者分为射血分数功能下降心衰组(C组)、舒张功能不全心衰组(D组)、收缩功能不全心衰组(E组)、混合性心衰组(F组),比较四组患者的超声心动图参数、MRI参数.运用Pearson相关性分析法,分析超声心动图参数与MRI参数的相关性.结果 超声心动图的EDV、ESV、SV、EF与MRI相近(P＞0.05);B组超声心动图参数及MRI参数中,EDV、ESV、SV均大于A组(P＜0.05),EF均小于A组(P＜0.05).四组患者的超声心动图参数及MRI参数中,EDV、ESV、SV、EF比较(P＜0.05);超声心动图参数中的EDV、ESV、SV、EF与MRI参数中的EDV、ESV、SV、EF均呈明显正相关.结论 超声心动图与MRI均能有效评估冠心病合并心衰患者的心功能,二者的左心功能参数存在明显相关性,具有较好的临床应用价值.
Abstract
Objective To compare the assessment effects between echocardiogram and magnetic resonance imaging(MRI)on heart function in patients with coronary heart disease-heart failure(CHD-HF).Methods CHD-HF patients(n=80)were chosen from Department of Ultrasound in Beijing Luhe Hospital affiliated to Capital Medical University from July 2020 to May 2023,and all patients were given examinations of echocardiogram and MRI.The parameters of left heart function[end-diastolic volume(EDV),end-systolic volume(ESV),stroke volume(SV),ejection fraction(EF)]were compared between echocardiogram and MRI.The patients were divided,according to NYHA classification criteria,into grades Ⅰ and Ⅱ group(group A)and grades Ⅲ and Ⅳ group(group B).The parameters of echocardiogram and MRI were compared between 2 groups.According to HF characteristics,the patients were divided into group of HF with reduced ejection fraction(group C),group of HF with diastolic dysfunction(group D),group of HF with systolic dysfunction(group E)and mixed HF group(group F).The parameters of echocardiogram and MRI were compared among 4 groups.The correlation between parameters of echocardiogram and MRI was analyzed by using Pearson correlation analysis method.Results EDV,ESV,SV and EF in echocardiogram parameters were similar to those in MRI parameters(P>0.05).In parameters of echocardiogram and MRI,EDV,ESV and SV were higher(P<0.05)and EF was lower(P<0.05)in group B than those in group B.In parameters of echocardiogram and MRI,the comparison in EDV,ESV,SV and EF showed P<0.05 in groups C,D,E and F.EDV,ESV,SV and EF in echocardiogram parameters were positively correlated to those in MRI parameters.Conclusion Echocardiogram and MRI all can effectively assess heart function in CHD-HF patients,and there is significant correlation between them in parameters of left heart function with higher application value in clinical practice.
