摘要

目的 建立基于光电容积法的便捷血管僵硬度检测技术,并评估临床诊断效能.方法 采用光电容积脉搏波描记法,结合便携式掌式12导联心电信号采集终端,建立动脉脉搏波传导速度的简便检测技术,应用于55例中国人民解放军总医院心血管内科住院患者,与现有动脉硬化检测技术比较,计算灵敏度、特异度、阳性预测值和阴性预测值等指标以及一致性检验.结果 可穿戴动脉硬化检测装置对动脉硬化检测的灵敏度、特异度及准确性分别为85.7%、95.2%、91.4%,且具有较好的一致性(Kappa值>0.80).结论 可穿戴动脉硬化检测装置具有便携、操作简单、快速测量的优点,在居家和社区中具有较好的应用前景.

Abstract

Objective To establish a convenient technique for detecting vascular stiffness based on photoplethysmography(PPG),and assess its clinical diagnostic efficacy.Methods A simplified technique for measuring arterial pulse wave velocity(PWV)was established by applying PPG combined with a portable handheld 12-lead electrocardiogram signal acquisition device.The simplified technique was applied in 55 patients hospitalized in Department of Cardiovascular Medicine of Chinese PLA General Hospital.A comparative analysis was conducted between the simplified technique and existing arterial stiffness detection technologies,and sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)were calculated,and the results were tested for consistency.Results The sensitivity,specificity,and accuracy of the wearable arteriosclerosis detection device was,respectively,85.7%,95.2%and 91.4%,with a great consistency(Kappa value>0.80).Conclusion The wearable device has the advantages of portability,simplification and quick in arteriosclerosis detection,and holds a promising application prospect in home and community.
