摘要
目的 探究高血压合并超重或肥胖患者的主要证候要素及靶标特征分布情况,以期对中医临床辨证论治提供理论基础以及为北京地区基层医院中医治疗高血压伴超重/肥胖患者提供帮助.方法 收集1102例高血压合并超重或肥胖患者的相关临床资料(包括71个症状、体征条目)建立Excel数据库,并导入SPSS统计软件进行降维因子分析,提取证候要素及靶标,并统计分布特点.结果 共提取得到15个公因子,归纳出7个证候要素:阴虚210例(25.15%)、气虚146例(17.49%)、阳亢136例(16.29%)、痰热98例(11.74%)、血瘀87例(10.42%)、痰浊82例(9.82%)、阳虚76例(9.10%),靶标包括脾、肝、心等多个脏腑,以脾为主,占35.58%.结论 高血压合并超重或肥胖患者主要证候要素为阴虚、气虚、阳亢、痰热、血瘀、痰浊、阳虚,病性为虚实夹杂,病位以脾、肝、心为常见.提示其治疗宜以滋阴、益气、潜阳、化痰、清热、活血、温阳为主,主张从脾论治,兼顾肝、心两脏.
Abstract
Objective To investigate the distribution of main pattern elements and target characteristicsin patients with hypertension complicated by overweight or obesity,in order to provide a theoretical basis for clinical diagnosis and treatment with traditional Chinese medicine(TCM)and to provide help for hypertensiontreatment with TCM in basic hospitals in Beijing.Methods The clinical data(including 71 symptoms and signs terms)were collected to establish Excel database in 1102 patientswith hypertensioncomplicated by overweight or obesity.All data were imported SPSS statistical software for analyzing dimensionality reduction factors,extracting syndrome elements and target organs and counting distribution characteristics.Results There were totally 15 common factors extracted and 7 pattern elements summed up,including yin-deficiency(in 210 cases,25.15%),qi-deficiency(in 146 cases,17.49%),yang-hyperactivity(in 136 cases,16.29%),phlegm-heat(in 98 cases,11.74%),blood-stasis(in 87 cases,10.42%),phlegm turbidity(in 82 cases,9.82%)and yang-deficiency(in 76 cases,9.10%).The target organs included spleen,liver and heart,and spleen was a dominant organ(35.58%).Conclusion The main pattern elements are yin-deficiency,qi-deficiency,yang-hyperactivity,phlegm-heat,blood-stasis,phlegm turbidity and yang-deficiency.The nature of disease is mixture of deficient and excess,and disease locations are commonly spleen,liver and heart in patientswith hypertensioncomplicated by overweight or obesity.It is suggested that the treatment should be based on nourishing yin,tonifying qi,tranquilizing yang,resolving phlegm,clearing heat,promoting blood and warming yang.Spleen should be considered as the first and foremost organ in the treatment,while liver and heart should also be taken into consideration.
