高功率环肺静脉电隔离常规联合高功率上腔静脉电隔离治疗阵发性心房颤动的临床效果

摘要
目的 探讨高功率环肺静脉电隔离常规联合高功率上腔静脉电隔离治疗阵发性心房颤动的有效性和安全性.方法 选取100例阵发性心房颤动患者,其中50例行单纯高功率环肺静脉电隔离术(对照组);另50例在高功率环肺静脉电隔离术基础上常规行高功率上腔静脉电隔离(试验组),比较两组手术操作时间、1年后心房颤动复发率及并发症发生情况.结果 高功率环肺静脉电隔离常规联合高功率上腔静脉电隔离的试验组较对照组手术操作时间延长(P＜0.05),但1年后心房颤动复发率降低(P＜0.05).两组各出现1例心包积液,试验组和对照组均未出现窦房结及膈神经损伤.结论 高功率环肺静脉电隔离常规联合高功率上腔静脉电隔离治疗阵发性心房颤动是安全的,且与单纯行环肺静脉电隔离相比可有效降低心房颤动术后1年的复发率.
Abstract
Objective To investigate the efficacy and safety of high-power circumferential pulmonary vein isolation(CPVI)combined with high-power superior vena cava(SVC)isolation in treatment of paroxysmal atrial fibrillation(PAF).Methods PAF patients(n=100)were chosen and divided into control group given only high-power CPVI and test group given routine high-power SVC isolation based on high-power CPVI(each n=50).The time of operation,AF recurrence rate 1 y later and incidence of complications were compared between 2 groups.Results The operation time was longer(P<0.05),and AF recurrence rate 1 y later was lower(P<0.05)in test group than those in control group.There was 1 case of pericardial effusion in each group,and there were no injuries of sinus node and phrenic nerve in 2 groups.Conclusion The high-power SVC isolation combined with high-power CPVI is safe in PAF treatment,and can effectively reduce AF recurrence rate 1 y after the operation compared with only high-power CPVI.
