5E模式全面管理对慢性心力衰竭稳定期患者康复作用的研究

摘要
目的 评估5E模式的全面管理对慢性心力衰竭(心衰)稳定期患者康复作用的临床效果.方法 选择2020年5月至2022年2月于阜阳市人民医院收治的慢性心衰稳定期患者106例作为研究对象,采用随机数字表法分为对照组(53例,常规管理),观察组(53例,基于5E模式的全面管理),比较两组患者干预后的临床效果.结果 干预前,两组自我效能感量表(GSES)和自我管理能力量表(ESCA)评分比较,差异均无统计学意义(P＞0.05).干预后,观察组的ESCA和GSES评分分别为(144.76±8.23)、(30.98±2.15)分均高于对照组的(120.97±5.38)分、(27.57±2.72)分,比较差异有统计学意义(P＜0.05);观察组的健康行为依从性高于对照组(P＜0.05);观察组的明尼苏达心力衰竭生活质量量表(MLHFQ)评分低于对照组,比较差异有统计学意义(P＜0.05).结论 基于5E模式的全面管理运用在慢性心力衰竭稳定期的患者中,不仅可增强患者自我保健意识,还可提高健康行为的依从性,从而改善生活质量.
Abstract
Objective To review the clinical efficacy of comprehensive management of 5E model(including encouragement,education,evaluation,exercise and employment)on rehabilitation in patients with chronic heart failure(CHF)during stable stage.Methods CHF patients during stable stage(n=106)were chosen from People's Hospital of Fuyang City from May 2020 to Feb.2022.All patients were divided,according to random digital table method,into control group(given routine management)and observation group(given 5E-based comprehensive management,each n=53).The clinical efficacy was compared between 2 groups after the interventions.Results The comparison in scores of General Self-efficacy Scale(GSES)and Exercise of Self-Care Agency Scale(ESCA)showed no statistical significance between 2 groups before the interventions(P>0.05).After the interventions,ESCA scores were(144.76±8.23)and GSES scores were(30.98±2.15)in observation group,and ESCA scores were(120.97±5.38)and GSES scores were(27.57±2.72)in control group(P<0.05).The compliance of health behaviors was higher in observation group than that in control group(P<0.05).The scores of Minnesota Living with Heart Failure Questionnaire(MLHFQ)were lower in observation group than those in control group(P<0.05).Conclusion 5E-based comprehensive management,applied in CHF patients during stable stage,can enhance the self-care consciousness,improve the compliance of health behaviors,and so as to improve quality of life.
