Objective To review systematically the accuracy of smartwatch in automatic diagnosis of atrial fibrillation(AF)and differences among different diagnostic techniques.Methods The databases of PubMed,EMbase and Cochrane Library were retrieved for collecting all studies on accuracy of smartwatch in AF diagnosis published before Dec.30,2022.According to inclusion and exclusion criteria,the literature was screened and data were extracted.The quality of included literature was reviewed by using tool of quality assessment of diagnostic accuracy studies(QUADAS-2),and a Meta-analysis was conducted by using Stata14 software.Results There were totally 12 literatures included with 5862 subjects.The results of Meta-analysis showed that,in automatic AF diagnosis with smartwatch,the combined sensitivity was 0.89(95%CI:0.80-0.94),combined specificity was 0.95(95%CI:0.91-0.97),positive likelihood ratio(PLR)was 18.15(95%CI:9.25-35.61),negative likelihood ratio(NLR)was 0.11(95%CI:0.06-0.22)and diagnostic odd ratio(DOR)was 160.76(95%CI:56.32-458.89).In automatic AF diagnosis with watch-electrocardiograph(w-ECG),the combined sensitivity was 0.79(95%CI:0.63-0.89)and combined specificity was 0.93(95%CI:0.81-0.97).In automatic AF diagnosis with photoplethysmography(PPG),the combined sensitivity was 0.96(95%CI:0.88-0.99)and combined specificity was 0.97(95%CI:0.92-0.99).Conclusion The sensitivity and specificity of smartwatch are higher in AF screening with great potential in AF diagnosis.
