Objective To investigate the influence of peripheral blood eosinophils(EOS)on short-term prognosis in patients with acute decompensated heart failure with reduced ejection fraction(HFrEF).Methods The patients with acute decompensated HFrEF were chosen for a prospective cohort study in Municipal Hospital of Zibo City from May 2021 to Jan.2023.All patients were given routine treatment and followed up for 6 months.The status of short-term prognosis(rehospitalization due to heart failure and all-cause mortality in 6 months)were recorded.The level of peripheral blood EOS was detected in the patients,and according to their quartiles,all patients were divided into Q1 group,Q2 group,Q3 group and Q4 group.The general data,and indexes of heart function at admission[N-terminal pro-brain natriuretic peptide(NT-proBNP),cardiac troponin Ⅰ(cTnⅠ),left ventricular ejection fraction(LVEF)]were compared among 4 groups.The short-term prognosis was compared among 4 groups.The influence factors of short-term prognosis were analyzed by using Logistic regression analysis,and influence of EOS on short-term prognosis was analyzed in patients with acute decompensated HFrEF.Results The differences in NT-proBNP,cTnⅠ and LVEF had statistical significance among 4 groups(P<0.05).The results of 6-month follow-up showed that in 117 patients,the rate of rehospitalization due to heart failure was 29.91%(35/117),all-cause mortality was 11.97%(14/117)and rate of poor prognosis was 34.19%(40/117).The rate of poor prognosis was higher in Q3 group and Q4 group than that in Q1group and Q2 group(P<0.05).The results of Pearson correlation analysis showed that EOS was negatively correlated to NT-proBNP and cTnⅠ(r<0,P<0.05),and positively correlated to LVEF(r>0,P<0.05).The results of single-factor Logistic regression analysis showed that the risk of poor prognosis increased in Q3 group and Q4 group compared with Q1group(P<0.05).After correcting gender,age,body mass index(BMI),hypertension,diabetes,hyperlipidemia,types of heart failure,NYHA classification of heart function,NT-proBNP,cTnⅠ and LVEF1 the risk of poor prognosis increased in Q2 group,Q3 group and Q4 group compared with Q1group(P<0.05).Conclusion EOS is an independent risk factor affecting short-term prognosis in patients with acute decompensated HFrEF.
