Ⅰ型鼓室成形术疗效与咽鼓管功能的相关性分析

摘要
目的 探讨慢性化脓性中耳炎(CSOM)Ⅰ型鼓室成形术的疗效与咽鼓管功能的关系.方法 对100例诊断为CSOM的患者行Ⅰ型鼓室成形术,根据术前咽鼓管功能测试结果分为咽鼓管功能正常组(对照组)和咽鼓管功能障碍组(观察组).术后随访 6 个月,记录并分析患者的听力结果和鼓膜愈合情况.结果 对照组鼓膜愈合率为 91.70%(44/48),观察组为 78.80%(41/52),2 组差异有统计学意义(P＜0.05).对照组和观察组术后的纯音听阈(PTA)和气骨导差(ABG)均低于术前(P值均＜0.05);2 组患者在术前及术后 6 个月的PTA和ABG差异均有统计学意义(P值均＜0.05),术前与术后的PTA差值和ABG差值的差异无统计学意义(P值均＞0.05).结论 咽鼓管功能状态对CSOMⅠ型鼓室成形术后患者的听力恢复无显著影响,但会影响术后的鼓膜愈合率.因此,在鼓室成形术前评估咽鼓管功能是很有必要的,有助于初步预测患者术后的恢复情况.
Abstract
Objective To investigate the relationship between the efficacy of type Ⅰ tympanoplasty and the function of the eustachian tube(ET)in chronic suppurative otitis media(CSOM).Methods One hundred patients diagnosed with CSOM all underwent type Ⅰ tympanoplasty who were divided into a dysfunctional group and a normal group according to the results of preoperative eustachian tube function test.The patients were followed up for 6 months after surgery,and their hearing outcomes and tympanic membrane healing were recorded and analyzed.Results The tympanic membrane healing rate was 91.70%(44/48)in the normal group and 78.80%(41/52)in the dysfunctional group,with a statistically significant difference between the two groups(P<0.05).The pure tone audiometry(PTA)and air-bone gap(ABG)of patients in the ET disorder group and the normal group were all lower than those before surgery(P<0.05).The PTA and ABG differences between the two groups were statistically significant(P<0.05)before surgery and 6 months after surgery respectively,but there was no significant difference in the difference of PTA or ABG before and after surgery between the two groups(P>0.05).Conclusions ET functional status does not have a great effect on hearing recovery after CSOM tympanoplasty,but it affects the healing rate of the tympanic membrane after surgery.Therefore,it is necessary to assess ET function before tympanoplasty to help us initially predict the patient's recovery after surgery.
