颅中窝入路内听道定位的研究进展

摘要
颅中窝入路是耳神经和侧颅底外科的常规手术入路,主要用于切除涉及内听道的各类病变.颅中窝入路的主要优势在于能完整暴露内听道,有利于术中直视下切除病变.精准定位内听道可有效避免行颅中窝入路时损伤耳蜗、前庭、半规管、面神经、听神经等重要结构.各种定位方法的灵活应用、完备的颞骨解剖知识、以高分辨率CT和磁共振成像(MRI)为基础的术中导航技术是精准定位内听道的关键.本文对颅中窝入路中内听道定位方法的研究进展作一概述.
Abstract
Currently middle cranial fossa approach is routinely used in the practice of neurotology and lateral skull base surgery to remove various lesions involving the internal acoustic canal.The main advantage of middle cranial fossa approach is its thorough exposure of the internal acoustic canal which can greatly facilitate lesion dissection under direct vision.To precisely locate the internal acoustic canal can effectively avoid damaging the cochlea,vestibule,semicircular canal,facial nerve,acoustic nerve and other important structures during the middle cranial fossa approach.Flexible application of various positioning methods,a wealth of knowledge of temporal bone anatomy,and intraoperative navigation technology based on high-resolution CT and MRI are the keys to accurately locate the position of internal acoustic canal.This article gives an overview of the methods of identifying internal acoustic canal in the middle cranial fossa approach.
