摘要

目的 构建头颈部鳞状细胞癌(HNSCC)的蛋白预后风险模型.方法 从癌症基因组图谱(TCGA)中下载HNSCC的蛋白表达数据和临床数据,筛选预后相关蛋白,评估蛋白质预后风险模型,分析其共表达蛋白.结果 NF2、TFRC、IRF1、INPP4B、PAX8、Granzyme-B和HEREGULIN被认为是特征蛋白.Granzyme-B、HEREGULIN、NF2 和PAX8 被纳入蛋白预后风险模型.HNSCC患者Kaplan-Meier生存分析,结果显示高风险组和低风险组患者预后差异具有统计学意义(P<0.001),在模型构建组中进行生存分析,结果显示模型构建组中高危组和低危组患者预后差异具有统计学意义(P<0.001),验证组中也得到了相同的结论(P=0.020).NF2、PAX8、HEREGULIN和Granzyme-B的表达与HNSCC患者的预后有独立相关性,通过COX单因素及多因素分析蛋白预后风险模型可以独立于其他临床性状进行HNSCC患者预后分析.蛋白预后风险模型的预测 1年、3年、5年生存期的曲线下面积分别为0.776、0.826、0.760,蛋白预后风险模型的独立预测能力明显好于其他临床性状.结论 本次研究构建的HNSCC蛋白预后风险模型具有良好的敏感性和特异性.

Abstract

Objective To construct a protein prognostic risk model for head and neck squamous cell carcinoma(HNSCC).Methods Protein expression data and clinical data of HNSCC were obtained from the Cancer Genome Atlas(TCGA).Prognosis-related proteins were identified,and the protein prognostic risk model was assessed along with analysis of its co-expressed proteins.Results NF2,TFRC,IRF1,INPP4B,PAX8,Granzyme-B,and HEREGULIN were identified as the characteristic proteins.Granzyme-B,HEREGULIN,NF2,and PAX8 were included in the protein prognostic risk model.Kaplan-Meier survival analysis showed significant differences in prognosis between high-risk and low-risk groups among HNSCC patients(P<0.001).Similar results were obtained in both the model construction group(P<0.001)and the validation group(P=0.020).The expressions of NF2,PAX8,HEREGULIN,and Granzyme-B were independently associated with the prognosis of HNSCC patients.COX univariate and multivariate analyses demonstrated that the protein prognostic risk model could independently predict the prognosis of HNSCC patients regardless of other clinical characteristics.The AUCs for predicting 1-year,3-year,and 5-year survival were 0.776,0.826,and 0.760,respectively,indicating that the independent predictive ability of the protein prognostic risk model was significantly better than other clinical characteristics.Conclusion The constructed HNSCC protein prognostic risk model exhibits good sensitivity and specificity.
