河南地区兔源克雷伯氏菌耐药性及耐药基因的分析

Analysis of antimicrobial resistance and related genes of rabbit-devived Klebsiella in Henan Province

为了解河南地区兔源克雷伯氏菌的生物学特性、携带的耐药基因及耐药基因与耐药表型之间的相关性,本研究于2022年3月~2023年4月从河南省7个不同地区规模化养兔场共采集74份具有典型克雷伯氏菌病临床症状兔的病料样品,分别划线接种于LB固体培养板和血琼脂培养板进行病原菌的分离培养.通过菌落形态观察、革兰染色镜检、生化试验、PCR扩增克雷伯氏菌Khe基因并经BLAST比对和构建Khe基因的进化树确定细菌的种类.结果显示,从74份兔病料样品中共分离到10株克雷伯氏菌,且该10株菌主要集中在洛阳(3/10)和周口(2/10),开封、郑州、漯河、济源及新乡各分离到1株.分别通过K-B法和PCR对不同地区来源的菌株进行3类共8种药物的药敏试验和上述药物的相关耐药基因检测,采用SPSS 26软件分析耐药表型与耐药基因之间的相关性.结果显示,对新霉素、庆大霉素、卡那霉素(氨基糖苷类),环丙沙星、诺氟沙星(喹诺酮类),四环素(四环素类)6种药物耐药的菌株占80%~100%,对氧氟沙星(喹诺酮类)和多西环素(四环素类)耐药的菌株占60%~70%,且多重耐药菌株占90%,并以耐庆大霉素和环丙沙星药物为主.对上述3类药物平均耐药菌株的占比分别为90%、80%和80%.从洛阳、开封和郑州地区分离的菌株耐药严重,对8种药物均耐药,呈严重的多重耐药性;周口和新乡地区分离的菌株耐药性虽相对较弱,但也呈多重耐药性.耐药基因的PCR结果显示,aac(6')-Ib、ant(3'')-I和aph(3'')-I(氨基糖苷类)、Aac6和qEpA(喹诺酮类)、tetQ(四环素类)的检测率均高达60%以上,qnrs(喹诺酮类)的检测率为30%,未检出tetO(四环素类).从洛阳分离的菌株携带耐药基因的数量和种类最多,从周口和郑州分离的菌株携带耐药基因的数量和种类较少,从开封、漯河、济源和新乡分离的菌株携带耐药基因的数量相同,但携带耐药基因的种类不同.经计算分析,10株菌对氨基糖苷类和喹诺酮类药物的耐药表型及其耐药基因之间基本呈正相关(符合率分别为81.4%和70.9%),四环素类耐药表型和耐药基因的符合率较低仅为37.5%,但分离菌对四环素类药物呈一定的耐药性.综上表明,河南地区兔源克雷伯氏菌的耐药性严重,且多呈多重耐药性,耐药基因较复杂多样.本研究首次检测了河南地区兔源克雷伯氏菌的耐药表型与耐药基因并分析它们之间的相关性,为河南地区兔场克雷伯氏菌病的防治提供参考依据.
To understand the biological characteristics,antimicrobial resistance genes and the correlation between the genes and resistance phenotypes of Klebsiella pneumoniae isolated from rabbits in Henan Province,in this study,74 rabbit samples with typical clinical symptoms of Klebsiellosis were collected from large-scale rabbit farm in 7 different areas of Henan Province from March 2022 to April 2023.The samples were inoculated on LB plates and blood agar plates,respectively,and the pathogenic bacteria were isolated and cultured.The bacterial species were determined by colony morphology observation,gram staining microscopy,biochemical test,PCR amplification of klebsiella Khe gene and BLAST comparison and construction of Khe gene phylogenetic tree.The results showed that a total of 10 Klebsiella strains were isolated from 74 diseased rabbit samples,and the 10 strains were mainly concentrated in Luoyang(3/10)and Zhoukou(2/10),and 1 strain was isolated in Kaifeng,Zhengzhou,Luohe,Jiyuan and Xinxiang,respectively.The drug susceptibility test of 8 kinds of drugs in 3 categories and the detection of 8 kinds of drug resistance genes in these 3 categories were carried out by the disk diffusion(K-B)method and PCR respectively,and the correlation between the phenotype and genes was analyzed by SPSS 26 software.The results showed that 80%-100%of the strains were resistant to neomycin,gentamicin,kanamycin(aminoglycosides),ciprofloxacin,norfloxacin(quinolones),and tetracycline(tetracycline),and 60%-70%of the strains were resistant to ofloxacin(quinolones)and doxycycline(tetracycline).Multidrug resistant strains accounted for 90%,mainly resistant to gentamicin and ciprofloxacin.The average proportion of strains resistant to 3 classes of drugs was 90%,80%and 80%,respectively.The strains isolated from Luoyang,Kaifeng and Zhengzhou were highly resistant to 8 kinds of drugs,showing severe multi-drug resistance.Although the resistance of the strains from Zhoukou and Xinxiang was relatively weak,they also showed multiple-drug resistance.PCR detection of drug-resistant genes showed that aac(6')-Ib,ant(3'')-I and aph(3'')-I(aminoglycoside),Aac6 and qEpA(quinolone class),tetQ(tetracycline class)detection rates were over 60%,qnrs(quinolone class)detection rate of 30%,and tetO(tetracyclines)was not detected.The strains isolated from Luoyang carried the most number and types of resistance genes,while the strains isolated from Zhoukou and Zhengzhou carried fewer.The number of resistance genes carried by strains isolated from Kaifeng,Luohe,Jiyuan and Xinxiang was the same,but types of resistance genes were different.After calculation and analysis,the aminoglycosides and quinolones resistance phenotypes of 10 strains were positively correlated with resistance genes(the coincidence rates were 81.4%and 70.9%,respectively).The coincidence rate of tetracycline resistance phenotypes and resistance genes was only 37.5%,but the isolates showed certain resistance to tetracycline.In conclusion,it demonstrated that rabbit-derived Klebsiella in Henan Province exhibit severe drug resistance,and most of them are multi-drug resistant,with complex and diverse resistance genes.This study explored the correlation between drug resistance phenotypes and drug resistance genes of rabbit Klebsiella in Henan for the first time,providing reference for the prevention and treatment of Klebsiella disease in rabbit farms in Henan.
