Objective To analyze the influencing factors of Malagasy people's willingness of active health-seeking,so as to provide reference for the formulation of China's foreign aid strategy. Methods The residents of Madagascar were anonymously surveyed using questionnaire interview to collect information the current status of willingness to actively seek medical care and potential influencing factors.Random forest model was used to rank the importance of the influencing variables, and based on the ranking results, the variables were selected using stepwise random forest analysis. Finally, the variables with the least out-of-bag error were included in a logistic regression model to estimate the direction and magnitude of the impact of the independent variables. Results A total of 461 Madagascar residents were included,of whom 371(68.76%)were willing to seek medical services actively. The results of random forest model and logistic regression model showed that living in rural areas was a negative factor inhibiting willingness of active health-seeking(OR=0.44,95%CI:0.28-0.69),while having infectious diseases(OR=2.04,95%CI:1.24-3.36),having access to medical staff information(OR=2.22,95%CI:1.43-3.45),receiving medical assistance(OR=2.28,95%CI:1.40-3.73),perceiving medical procedures as convenient and prompt(OR=2.30,95%CI:1.22-4.34),being moderately satisfied(OR=3.09,95%CI:1.59-5.99)or satisfied with health services(OR=3.48,95%CI:1.64-7.38),and having experienced full recovery after medical care(OR=3.05,95%CI:1.41-6.59)were positive factors promoting willingness to actively seek medical care. Conclusions Some of the Malagasy people still have low willingness of active health-seeking,for whom more guidance and improvement are necessary. The findings of this study suggest that China and Madagascar should strengthen cooperation and exchange in key areas and jointly promote the rapid development of health care on the Madagascar side.
