Objective To investigate the vaccination willingness and vaccination rate of 23-valent pneumococcal polysaccharide vaccine(PPV23)and the influencing factors among the elderly population in Guangzhou,so as to provide evidence for formulation of pneumonia vaccination strategy for the elderly in Guangzhou City.Methods A survey was carried out in 11 districts of Guangzhou from July 10 to 31,2022,selecting the elderly people aged 60 and above as the survey subjects.Multivariable logistic regression was used to analyze the factors associated with the vaccination willingness and vaccination rate of PPV23 among the elderly people.Results A total of 1 316 survey subjects were included,of whom 1 157(87.92%)reported as willing to be vaccinated with PPV23,and 186(14.13%)had been vaccinated with PPV23.The main reasons that the elderly were willing to receive PPV23 were the effectiveness of the vaccine in reducing the possibility of pneumonia(82.80%),the safety and reliability of the vaccine(60.67%),and recommendation of clinicians(47.28%).The elderly were more concerned about whether the pneumonia vaccine was effective(77.81%),whether the vaccine was safe(76.44%),and whether they were suitable for vaccination(68.62%).They preferred to receive information about pneumonia vaccine from staff of vaccination clinics(64.06%),TV or radio(54.18%)and clinicians(51.22%).The results of multivariable analysis showed that the elderly people having had fever with cough and sputum within the past year(OR=1.68,95%CI:1.09-2.59),had previous vaccination against influenza(OR=2.99,95%CI:1.76-5.09),and those having good awareness of pneumonia(OR=2.41,95%CI:1.58-3.68)and pneumonia vaccine(OR=4.37,95%CI:2.83-6.72)were more willing to receive PPV23.Older age(OR=2.43,95%CI:1.42-4.16),having had previous vaccination against influenza(OR=4.27,95%CI:2.96-6.17),and having fair awareness of pneumonia vaccine(OR=3.81,95%CI:2.44-5.93)were associated with higher PPV23 vaccination rate among the elderly people.Conclusions The elderly in Guangzhou have a high willingness to be vaccinated with PPV23,but the vaccination rate is low.Strengthening health education on the effectiveness and safety of pneumonia vaccines,encouraging medical professionals to recommend vaccination to the elderly,and improving the accessibility and convenience of vaccination services for the elderly may help to elevate the vaccination rate of pneumonia vaccines in the elderly population.
