Research on the knowledge,attitude,and practice of community family doctors towards the integrated medical and care service model in Nanjing and its influencing factors
Objective The aim of this study is to understand the knowledge,attitude,and practice of family doctors towards the integrated medical and care service model and to identify its influencing factors in order to provide a basis for further improvement of integrated medical and care services.Methods In April 2023,using a systematic sampling method,a survey was conducted on 310 family doctors from 7 community health service centers in Jiangning District,Nanjing.The survey included basic information about the family doctors and their knowledge,attitudes,and behaviors toward the integrated medical and care service model.Data analysis was conducted using SPSS 26.0 statistical software.The chi-square test was used for univariate analysis,and Pearson correlation analysis was used to assess the correlation of knowledge,attitude,and practice scores.A cut-off value of 60%was used to judge the performance of family doctors in terms of knowledge,attitudes,and behavior scores.Multivariate analysis was performed using binary logistic regression.Results The total score for the knowledge,attitude,and practice of family doctors towards the integrated medical and care service model was(43.08±3.28),with a scoring rate of 53.85%.There was a positive correlation between knowledge score and attitude score(r=0.13,P=0.02)and a positive correlation between attitude score and behavior score(r=0.31,P<0.01).Family doctors with 11-<21 years of work experience demonstrated better knowledge scores(OR=3.287,95%CI:1.75-6.16);those with a college education or less(OR=8.82,95%CI:1.64-47.48)and those with a bachelor's degree(OR=5.53,95%CI:1.12-27.30)demonstrated better attitude scores;and those with an income of 0-<5 001 yuan(OR=0.30,95%CI:0.09-0.96)and an income of 5 001-<8 001 yuan(OR=0.37,95%CI:0.14-0.97)performed poorly in terms of attitude scores;and family doctors employed under public institutions showed better practice scores(OR=2.46,95%CI:1.08-5.60).Conclusions The knowledge,attitude,and practice scores of family doctors towards the integrated medical and care service model are relatively low.Targeted measures should be taken according to the main factors that influence the motivation of family doctors with different work experiences,educational backgrounds,incomes,and employment statuses in order to promote the smooth implementation of integrated medical and care services.
