Assessment of the health system in Guinea and recommendations for China's medical assistance
Objective To understand the structure of Guinea's health system and the national health status and to provide policy recommendations for China's targeted medical assistance to Guinea.Methods Documents and public data from the health administrative departments and health institutions in Guinea were collected,and relevant personnel were interviewed.The structure and current status of the Guinean health system were described according to the"six elements of health system"framework developed by the World Health Organization.Results Communicable diseases are still the primary disease burden and cause of death in Guinea,with malaria having the highest incidence rate(152.43‰)and mortality rate(15.39/100 000).The structure of the Guinean health system is organized at the central,regional,and provincial levels,each of which has a corresponding health administration.Health service institutions are categorized as public,semi-public,private,and community-based according to their nature.The main sources of health expenditure include the national and local governments,individuals,partners,the private sector,donors,and non-governmental organizations,with individual payments accounting for the highest proportion(64.25%).The total number of health professionals nationwide is 10 553,with the largest proportion being healthcare workers(3 985,37.76%).The total number of beds nationwide is 2 984,with an average of one bed per 3 600 people,and the unavailability of basic medical equipment is 70.00%.The national health information system consists of eight subsystems that contribute to statistics and reports of essential health information.Conclusions Guinea has established a relatively complete health system framework.However,serious problems still exist,such as insufficient government health expenditure,insufficient health human resources,lack of health supplies,chaotic management of medicines,and a low level of informatization.China's medical assistance to Guinea should be strategically planned in the long term,expand the coverage of assistance,and promote the diversification of modalities.
