不可修复肩袖撕裂的诊治研究进展

摘要
肩袖撕裂是最常见的肩部损伤之一.对于有活动需求的肩袖撕裂患者,解剖性肩袖修复手术是首选治疗策略.然而,部分患者难以实现肩袖撕裂的解剖修复,其治疗策略对肩关节外科医师具有挑战性.不可修复肩袖撕裂的手术治疗方式选择较多,包括单纯肩关节清理或联合肱二头肌长头腱切断或固定、部分肩袖修复、(反向)肩峰下减压手术、肌腱转位术、肩峰下球囊植入、上关节囊重建术、肩袖桥接重建术及反置式人工全肩关节置换术等.本文作者通过回顾相关文献,以临床证据为基础,总结了针对不可修复性肩袖撕裂的治疗策略.
Abstract
Rotator cuff tear is one of the most common shoulder injuries.For patients with functional demands,anatomic rotator cuff repair surgery is considered the most-preferred treatment.However,some patients are not suitable candidates for anatomical repair of rotator cuff tears,posing challenges for shoulder surgeons in determining the appropriate treatment strategy.Various surgical options exist for irreparable rotator cuff tears,including arthroscopic debridement alone or in combination with biceps tenotomy or tenodesis,partial rotator cuff repair,reversed subacromial decompression,tendon transfers,subacromial biodegradable spacer implantation,superior capsular reconstruction,rotator cuff bridging reconstruction,and reverse total shoulder arthroplasty.In this paper,the authors review relevant literature and summarize treatment strategies for irreparable rotator cuff tears based on clinical evidence.
